Purpose:In this experiment,a spinal cord injury model was used as the experimental object.The urodynamics,spinal cord and bladder tissue morphology of the spinal cord injury model rats were examined to observe the therapeutic effect of acupuncture at "Jiaji" acupuncture point combined with low-frequency electrical stimulation.The expression of PACAP in the spinal cord and bladder detrusor was detected by ELISA method;the expression of c AMP and PKA in bladder detrusor and spinal cord were detected by ELISA method.Based on the PACAP-c AMP-PKA signal pathway,acupuncture at "Jiaji" acupuncture point combined with low-frequency electrical stimulation Mechanism of action.Provide theoretical basis for future clinical treatment of urinary retention after spinal cord injury.Material and method:In this experiment,50 healthy female SD rats were selected and randomly grouped into blank group,sham operation group,model group,treatment group(acupuncture combined with low frequency electrical stimulation)and low frequency control group,ten rats in each group.Blank group: no intervention;sham operation group: only bite the lamina without performing spinal cord blow;model group: use Allen's spinal cord blow method for modeling without taking any treatment measures;low frequency control group:perform Allen's blow,blow The model that caused spinal cord injury and urinary retention was treated 24 hours later.The treatment site is divided into two groups,a total of 4 electrode pads: the first group of 2 electrode pads,the negative electrode of the electrode pad is placed on the bladder area of the rat pubis,and the positive electrode pad is placed on the third sacral vertebrae of the rat;The second group of 2 electrodes is placed at the Jiaji points of the upper and lower segments of the spinal cord injury,the positive level is on the top,the negative electrode is on the bottom,the output waveform is a triangle wave,low frequency electrical stimulation(50Hz),according to the maximum current intensity does not exceed 50 m A,to appear Moderate muscle contraction is degree,20 min / time,1 time / d,continuous intervention for 7 days;treatment group: acupuncture three pairs of Jiaji points on the basis of low frequency electrical stimulation to locate bilateral T6,T9,T11 "Jiaji " Acupuncture point,the acupuncture slanted into about 2mm,left the needle for 20 min,no acupuncture and no diarrhea during the acupuncture,no electricity,once a day,continuous treatment for 7 days.After treatment,the urodynamics of each group of rats were tested.The spinal cord,bladder and detrusor muscle of each group were taken.(1)HE staining method was used to detect the morphological changes of rat spinal cord and bladder detrusor muscles;(2)Immunohistochemical method was used to observe the expression of PACAP-38 protein in rat spinal cord and bladder detrusor muscles;The level of c AMP and PKA in the spinal cord and bladder detrusor of rat was measured by immunosorbent,and statistical analysis was performed.Results:1.Acupuncture at Jiaji points combined with low-frequency electrical stimulation can obviously improve the maximum bladder capacity and bladder compliance of urinary retention model rats after spinal cord injury,reduce the bladder pressure and restore some bladder functions.2.Acupuncture at Jiaji points combined with low-frequency electrical stimulation can promote spinal cord repair,reduce the degree of pathological damage to the bladder,and improve the tissue morphology of the spinal cord and bladder detrusor in urinary retention model rats after spinal cord injury.3.Acupuncture at Jiaji points combined with low-frequency electrical stimulation can increase the expression of PACAP protein in the spinal cord and bladder detrusor muscles of urinary retention model rats after spinal cord injury,increase the content of c AMP and PKA in the spinal cord and detrusor muscles,thereby repairing the spinal nerves.Induces detrusor relaxation and improves bladder function.4.Acupuncture at Jiaji points combined with low-frequency electroacupuncture by activating the PACAP-c AMP-PKA signaling pathway may be an important way for acupuncture to reconstruct urinary retention bladder function after spinal cord injury.Conclusion:1.Acupuncture combined with low-frequency electrical stimulation can significantly improve the maximum bladder capacity and bladder compliance of urinary retention model rats after spinal cord injury,reduce intravesical pressure,and restore some bladder functions.2.Acupuncture combined with low-frequency electrical stimulation can promote spinal cord repair,reduce the degree of pathological damage to the bladder,and improve the morphology of spinal cord and bladder in urinary retention model rats after spinal cord injury.3.Acupuncture combined with low-frequency electrical stimulation can increase the expression of PACAP protein in spinal cord and bladder detrusor muscle of urinary retention model rats after spinal cord injury,increase the content of c AMP and PKA in spinal cord and detrusor muscle,thereby repairing spinal nerves and inducing detrusor Muscle relaxation,improving bladder function.4.Acupuncture combined with low-frequency electroacupuncture through activation of the PACAP-c AMP-PKA signaling pathway may be an important way for acupuncture to reconstruct urinary retention bladder function after spinal cord injury.
