Objective:According to the clinical studies and animal experiments prove the effectiveness of cortex phellodendri which to treatment the venous leg ulcer,and then further research the influence of NF-KB and TNFAIP3/A20 in the wound healing.Methods:The 48 cares of lower limb vein ulcer patients was divided into treatment group.control group and blank group by stochastic averaging.The treatment group was given water solution of cortex phellodenri on the ulcer foucs which was intravenous drip by daxin ligustrazine injection.The treatment group was given water solution of cortex phellodenri external treatment on the ulcer foucs after the intravenous drip by daxin ligustrazine injection.The control group was given compound cortex phellodendri liquid external treatment on the ulcer focus after the intravenous drip by daxin ligustrazine injection.The blank group was given the intravenous drip by daxin ligustrazine injection.Observe the surface color of the ulcer,the area size of the ulcer and the change of the healing time.Experimental research and innovation established the model of inferior limb venous ulcer.Apply the cortex phellodendri liquid to the ulcer of the rat.The control group of the positive drug chose to the compound cortex phellodenri liquid.The serum IL-1beta,IL-6 and TNF-? were detected by ELISA method.The RT-PCR method is used to detect the expression of NF-KB?IKB and TNFAIP3mRN A in the ulcer tissue.The WB method is used to detect the expression of NF-KB?IKB and TNFAIP3 in the ulcer tissue.Results:Clinical research:The water solution of cortex phellodendri treat sixteen cases of the lower limb venous ulcers.Five cases were cured,5 cases were developed and 1 was improved.The total efficiency of the treatment is 68.75%.The control group treated sixteen cases of venous ulcers.Six cases were cured,four cases were developed and two cases were improved.The total efficiency of the treatment is 75%.The blank group treated sixteen cases of venous ulcer.Three cases were cured,four cases were developed and three cases were improved.The total efficiency of the treatment is 62.5%.1.The treatment group compared with the blank group to the total effective rate have obvious difference(P<0.05).The results prove that the water solution of cortex phellodenri is a real and effective treatment for lower limb venous ulcers.2.Compared with the control group,the total efficiency of the treatment group was obviously different(P<0.05),which proved that the treatment effect of Huangbaiye was better than that of the decoction.3.The treatment group compared with the blank group to the ulcer surface colour,wound granulation tissue,ulcer area size and the change of the healing time has obvious difference(P<0.05);There was no significant difference between treatment group and control group(P>0.05).Experimental study:1.Innovation established the model of venous ulceration of the lower limb of rats in this experiment.After the rat model was established that appeared the lower extremities swollen and the skin is dark red,and the ulcerate is exuded with suppurated secretions,which reduces the amount of food and water,less dynamic and hollow back.The control group and the treatment group were significantly better than the surgical group.2.HE staining under light microscope microscopic changesSham-operated group 7days:The cells are spindle,and the cells are arranged in reat and uniform size.Model group 7 days:Inflammatory cells appear in large numbers,tissue edema is evident,red blood cells are apparent,tissue cell structure is loose.Treatment group 7 days:Tissue edema begins to subside,inflammatory cells decrease,connective tissue begins to proliferate.Control group 7 days:Tissue edema begins to subside,inflammatory cells was significantly reduced,and the connective tissue hyperglycemia was evident.Control group 14 days:Edema disappears,inflammatory cells disappear,connective tissue hyperglycemia.3.The level of IL-1??IL-6?and TNF-? is detected in the wound tissue.After the model was established,the three levels increased significantly over time.Three was significant reduction in the treatment group and the control.4.TNFAIP3/A20 activated the IKB to inhibit the NF-KB pathway and then reduce the inflammation which to achieve the goal of the treatment of lower limb venous ulcer.The experiment provides a new ideas and approaches.5.The water solution of cortex phellodendri increase the express of TNFAIP3/A20 to inhibit the express of NF-KB,and to effective treatment the rats of the lower limb venous ulcers.Conclusion:Clinical research:The total efficiency of the treatment is 68.75%.Experimental study:1.Innovation established the model of venous ulceration of the lower limb of rats in this experiment.2.Through measure the content of IL-1??IL-6?TNF-? and the variation of the surface of the rat to prove the successful established of the model of venous ulceration of the lower limb of the rats.Further verified to the water solution of cortex phellodendri which can effective treatment for lower limb venous ulcers by reduce the content of the IL-1 ??IL-6 and TNF-?.3.The level of NF-KBmRNA dropped,and the level of IKB?TNFAIP3/A20 ascend to participate in the healing process of the lower limb venous ulcer.4.TNFAIP3/A20 activate the IKB to inhibit the NF-KB pathway and then reduce the inflammation which to achieve the goal pf the treatment of lower limb venous ulcer.The experiment provides a new ideas and approaches.5.The water solution of cortex phellodendri increase the express of TNFAIP3/A20 to inhibit the express of NF-KB,and to the rats of the lower limb venous ulcers effective treatment.6.Compared with Compound Cortex Phellodendri,the results showed no significant difference between them(P>0.05).
