ObjectiveChronic renal failure(CRF)has become a common and difficult clinical disease,with an incidence of about 1 in 10,000,accounting for 10%of the mortality of the population.At present,there is no significant breakthrough in modern medical treatment,which can only be achieved through hemodialysis,peritoneal dialysis and kidney surgery transplantation in the late stage.It is often stopped due to the great side effects,poor effect,high price and patients’ inability to adhere to western medicine.Traditional Chinese medicine believes that kidney essence can nourish the five internal organs,and that deficiency of vital qi and marrow will lead to chronic kidney disease,and its clinical manifestations are different.However,there are common etiology and pathogenesis of the disease.Traditional Chinese medicine has unique advantages in the prevention and treatment of CRF,which can help to delay the progression of chronic kidney disease.In this study,SD rat model of chronic renal failure was adopted to observe its intervention effect on chronic renal failure through acupuncture and drug intervention combined with acupuncture at shenshu point and AH888,which has great clinical significance and innovation.MethodsFifty-nine six-week-old SD rats were randomly divided into four groups according to a random number table method:blank group,model group,drug group and acupuncture-drug combination group,of which 12 rats were in blank group,16 rats in model group,15 rats in drug group and 16 rats in acupuncture-drug combination group.After the rats were adaptively fed for 7 days.First days groups of model group,drug group and acupuncture-drug combination group were given a single intravenous injection of(4.5 mg/kg)adriamycin,and then given daily adenine gavage(260 mg/kg.d),and the equal volume of sodium chloride blank group.Intravenous saline injection and intragastric administration were done in blank group as well as control group.Intravenous administrated for 16 consecutive days adenine,was(260 mg/kg.d).For induction of renal injury.Urine and blood were collected on the 16th day in model group,drug group and acupuncture-drug combination group,and routine urine and blood analysis were performed.The relevant data in the treated groups were compared with group Blank groups and the significant difference was considered at P<0.05.One rat was randomly selected,and the kidneys were sectioned for the pathological examination for the lesion grading of CKD in rats.The treatment was started after the confirm of the establishment of the CKD model.The dose of AH888:was 310 mg/kg in drug group(based on the human daily recommended intake dose as 3g).Acupuncture rats were treated with ShenShu points.Leave the needle for 15 minutes(once every 2 days for a total of 7 treatments).Urine and blood were collected and collected on the 5,10,and 15 days,and routine urine and blood analysis were performed to analyze serum BUN and Cre,and 24-hour urine samples were collected for urine protein measurement.Blank groups and modelgroups were also given an equal volume of 0.9%physiological saline.After 15 days of continuous gavage treatment in each group,all kidney samples were collected at the end of treatment.Sacrificed after anesthesia with isoflurane,the left kidney of the rats was frozen in a refrigerator at-200 C.The right kidney was removed in 10%formalin for one week.The tissue sections were embedded in paraffin,and stained with H&E method for the pathological examination.ResultsDuring the modeling process,the weight of rats in the model group,drug group and acupuncture-drug combination group was significantly higher than that in the blank group,suggesting pathological manifestations such as hydronephrosis in the modeling process.The drinking water intake and feed intake of rats in drug group and acupuncture-drug combination group were significantly higher than those in model group(P<0.001).Compared with the model group,the drug group and the acupuncture-drug combination group can obviously reduce the serum creatinine and urea nitrogen levels in the treatment process(P<0.001).15 days after treatment,compared with the model group,the total serum protein and serum albumin in the drug group and the acupuncture-drug combination group increased(P<0.001).Compared with the drug group,the acupuncture and drug combination group can significantly reduce urinary protein(P<0.001).Acupuncture or combined drugs can reduce serum phosphorus level and increase serum calcium ion level in CRF rats(P<0.01).The combination of drugs and acupuncture did not significantly improve serum cholesterol and potassium levels in CRF rats(P>0.05).At the same time,the research results show that the combination of drugs and acupuncture can reduce the weight of both kidneys of CRF rats,reduce the discharge of part of hydronephrosis,and increase renal blood flow(some blood streaks are observed in the kidney in the tissues taken).Both groups have the effect of protecting kidney.The kidneys of the drug group and the combination group were more ruddy.HE pathological staining showed that both groups had the effect of protecting kidney and could reduce adenine crystal accumulation in kidney.The survival rate of rats in each group is higher during the test,but the survival rate of rats in the drug group and the combination of acupuncture and medicine is higher than that in the model group.Further analysis found that needle-drug combination can reduce serum creatinine level,retain serum total protein,reduce proteinuria in CRF rats and increase urinary creatinine excretion compared with drug group.It is suggested that acupuncture combined with medicine can promote the elimination of metabolic wastes in CRF rats and keep the total serum protein to achieve the effect of protecting kidney.It is indicated that acupuncture combined with medicine can be used to treat CRF lesions clinically,and acupuncture combined with medicine can promote each other.ConclusionThis experiment proves that the drug group and the acupuncture-drug combination group have the effects of improving renal function of CRF model rats,reducing urinary protein excretion,reducing serum creatinine,serum urea nitrogen,etc.The combination of acupuncture and medicine can reduce serum creatinine level,retain serum total protein,reduce urinary protein in CRF rats,and increase urinary creatinine excretion.The results showed that the mycelia of Acremonium combined with acupuncture at Shenshu point of rats had good therapeutic effect on CRF of rats,which was worthy of further study.
