OBJECTIVE: To analyze the skeletal morphology,kinematics,and biomechanical characteristics of flatfoot boys' feet,and to study the effect and mechanism of orthopaedic insoles and exercise intervention methods,and then to provide theretic basis for the flatfoot boy's treatment.METHODS: 9-10 year old flat boy(28,divided into 4 groups: blank group(F0),orthopaedic insoles group(F1),exercise intervention group(F2)and orthopedic insole-exercise intervention group(F3),recruited 6 Normal foot(N0)boys participated in the experiment,and there was no significant difference in height and weight.The Qualisys infrared motion capture system,Kistler force table,Delsys electromyography,and plantar pressure plate were used to obtain the subject's date of natural walking kinematics,biomechanics,myoelectric,and plantar pressure.Application of digital X-ray test system,muscle color ultrasound test system and LorAn plantar foot 2D scanning system to obtain the subject's foot date and muscle shape date.The subject's flexor dorsiflexion force was measured using a Micro digital intensometer.The experimental group was intervened for 4 months and then tested again.Through Visual 3D,Matlab and other data analysis software to process the data obtained from the test,and use SPSS software in one-way analysis of variance,two-factor repeated measurement analysis of variance,statistical significance level of p <0.05.Results:(1)The long-term intervention had a good subjective effect on the flat-footed boys,especially after 2 months of wear.(2)Flat-footed boys' skeletal arch,talus,sacrum,and distance from the boat.The joint and Achilles tendon joints had specific features.After intervention,they were obviously normal.The effect of F3 was most obvious(foot scaphoid height increased by 4.41 mm,talus height increased by 2.93 mm,and angle from boat joints decreased by 4.53°),followed by F1.(3)The morphological characteristics of tibialis anterior muscle,tibial tendon muscle and tibialis longus muscle in flatfoot boys have unique characteristics.After intervention,they tend to normal foot.The effect of F3 is the most obvious(cross-sectional area of tibial muscle increases 0.19cm2,tibialis anterior muscle transverse,the cross-sectional area increased by 2.68cm2),followed by F2.(4)The gait characteristics,kinematics,and dynamics of the flat-footed boy had specific characteristics.After the intervention,it obviously tended to normal foot.The effect of F3 was most obvious(the bow index decreased 5.02%,arch pressure increase 1.31N/cm2,pace increase 0.21m/s,A1 M increase 0.078Nm/kg,A2 M increase 0.398Nm/kg).Conclusions: 1.Subjective sensory thresholds were found in two-months-old boys with orthopaedic insoles and exercise intervention.2.Orthopaedic insoles can effectively improve the performance of the foot scaphoid,talus,zygomatic bones,arches and ankle joints of flatfoot boys.3.Exercise intervention can effectively improve the muscle mass and knee joint performance of the tibialis anterior muscle,the abductor pollicis muscle,and the tibialis longus muscle in the flatfoot boy.4.The combined use of orthopaedic insoles and exercise interventions is better for improving the flat feet of boys.
