Purpose: The purpose of this study was to compare the effects of different custom orthopedic insoles on the biomechanical characteristics of high-arched during running.Evaluate the advantages and disadvantages of each insole in order to find the orthopedic insole that is most suitable for high-arched.It provides a reference for the design of high arch orthopedic insole,and provides a theoretical basis for clinical rehabilitation and treatment of high-arched and prevention of injury.Method: 9 high-arched with forefoot eversion and shin splints were screened.According to the information of high-arched,9 normal arch were matched.Designed three pairs of insoles for each high arched subject,respectively arch support insole(AS),arch support adds forefoot lateral wedge insole(AS+FW)and forefoot lateral wedge insole(FW).The biomechanical characteristics of the lower limbs during running in four states of the high-arch and the normal arch were collected by using a motion capture system,a three-dimensional force table,and pressure insole.The independent sample T-test was used to compare the differences between the normal arch and each state of high-arched.One-way repeated measure ANOVA were used to investigate differences between different state of high-arched.P<0.05 was significant difference.Result: When the high-arched runs,the contact surface is small,and the peak force,peak pressure and pressure impulse are larger than the normal foot.When the contact,the rearfoot dorsiflexion angle and lateral rotation angle are smaller than the normal foot.In the stand stage,the rearfoot of the high arches showed excessive internal rotation and insufficient lateral rotation.When running off,the rearfoot internal rotation and the midfoot flexion were larger.The first peak force and loading rate are significantly greater than the normal foot runner.High-arched can increase the foot contact area when wearing three pairs of soles,but the sole pressure is greater than the normal foot.The force of the two pairs of insoles with the forefoot lateral wedge insole is lower than that of the high arch without the insole.When running with three pairs of insoles The maximum pressure is significantly smaller than when there is non-insole,and the pressure impulse is less than that of the non-insole when wearing two pairs of insoles with arch support.Wearing the AS insole and the FW insole has a dorsiflexion angle less than that of the normal foot,and the AS+FW insole have a smaller lateral rotation angle than the normal foot.The maximum dorsiflexion angle of the ankle of the two pairs of insoles with the forefoot lateral wedge insole was smaller than that of the normal foot.The internal rotation of the ankle and rearfoot was larger than the normal foot,and the peak of the ankle and rearfoot internal rotation was smaller than the normal foot.Wearing the AS insole knee flexion and extension and the midfoot and lateral rotation of the midfoot are larger than the normal foot,and the ankle inversion and eversion is smaller than the normal foot.The angle of the ankle and rearfoot internal rotation of the AS+FW insole is significantly greater than the normal foot when running off.The first peak of the three pairs of insoles appeared late,the loading rate was lower than the non-insole,but the AS insole had a higher loading rate than the other two pairs of insoles.Conclusion: 1.Compared with the normal foot the high-arched of the arch is more concentrated,the cushioning capacity is poor and the incidence of shin splints,ankle injury and lower limb stress injury is greater.2.High-arched wearing three pairs of insoles can disperse the pressure,increasing the cushioning capacity by reducing the loading rate.The AS+FW insole is the best one to disperse the pressure.3.The high archer wearing the AS+FW insole can increase the dorsiflexion angle of the rearfoot when running,but the rotation angle increases during the stand period and increasing the knee load.Wearing the AS insole knee joint flexion and extension and the midfoot and external rotation of the midfoot is large,and can inhibit the high-arched when the foot is over-spinning when running off the ground,can prevent the tibial stress syndrome.4.High-arched wearing three pairs of insoles can increase cushioning capacity by reducing loading rate,arch AS+FW insole is the best insole to reduce loading rate.
