Background:Child's Hanmian is a commonly encountered disease,which affects child's constitution?intelligence and personality,bringing about slowness of growth and development?cognitive disability and mental activity abnormality,even sudden and unexpected death.The parents and medical workers must pay close attention to child's Hanmian.At present,the research on child's Hanmian is rare.Mostly surgical removal of tonsil and adenoid is used to cure child's Hanmian.But surgical removal of tonsil and adenoid would bring about loss of immune function partially,some children and parents have fears of operation and anesthesia,and some families can not afford to pay surgical operation cost. So it is imperative to seek a safe?effective and low-cost therapy method.The tonsil-pecking method is based on traditional external therapy,it is effective on treating child's Hanmian and easily accepted by children and parents.Objective:This project was based on randomized controlled trial.The child's Hanmian was treated with tonsil-pecking method.The level of CRP in blood serum and TNF-?in blood plasma of ten children in treatment group was tested. The clinical efficacy of tonsil-pecking method on child's Hanmian was observed and potential mechanism was explored.Method:67 patients were divided randomly into two groups—treatment group and control group.The treatment group with 34 patients was treated with tonsil-pecking method,the control group with 33 patients was treated with azithromycin orally.We observed symptom-and-sign score?individual symptom and physical sign?volume changes of tonsil and adenoid?ratio of A/N and measured the level of CRP and TNF-?in blood serum of ten children in treatment group.Result:During the whole study 64 cases were under study,3 cases were discontinued due to their abnormalities.The level baseline of the two groups was similar.The efficacy rate achieved in treatment group was 84.38%,compared with control group 56.25%.The result revealed statistical significant differences between two groups.The total symptom-and-sign score of every group had decreased.There were statistical significant differences between two groups on symptom improving of snore?sleep apnea and waking up by blocked,but no difference on mental state and dryness of mouth.Comparing with the pre-therapy,the tonsil and adenoid diminished obviously in treatment group,but changed hardly in control group.The ratio of A/N on adenoid in treatment group was improved,there were few changes in control group.The level of CRP and TNF-?in blood serum of ten children in treatment group lowered significantly after treatment.Conclusion:1.The tonsil-pecking method on child's Hanmian was effective by reducing the volume of tonsil and adenoid to relieve airway obstruction.2.The level of CRP and TNF-?in blood serum of ten children in treatment group lowered after treatment,which showed systemic inflammation in treatment group was relieved with tonsil-pecking.The pathological and physiological action of inflammation and anti-inflammatory therapy in child's Hanmian need further discussion.3.The immune function would be bosted by tonsil-pecking.4.The child's Hanmian with chronic tonsillitis and adenoid hypertrophy should be firstly treated by tonsil-pecking method.If the main symptoms are improved obviously,the operation on tonsil and adenoid should be canceled, venture and complication of operations and anaethesia would be avoided.5.The tonsil-pecking method on child's Hanmian is safe and effective,which break a new path for treating child's Hanmian.This therapy method is worthy of being popularized and practised widely as an external treatment of Traditional Chinese Medicine.
