Study On The Rule Of Acupoint And External Medicine Selection And Systematic Review Of External Therapy Of Traditional Chinese Medicine In The Treatment Of Vertigo

Objective: To sort out the development source of vertigo in the treatment of vertigo with external treatment of traditional Chinese medicine,analyze the rules of acupoints and medicine selectionin the treatment of vertigo with external treatment of traditional Chinese medicine,and explore the theoretical basis of external treatment of traditional Chinese medicine in the treatment of vertigo.A systematic review and meta-analysis of the effectiveness and safety of TCM external treatments for vertigo were conducted to provide evidence-based medical evidence for TCM external treatments for vertigo.Methods: a.Before the founding of the People's Republic of China,literature retrieval of Dictionary of Traditional Chinese Medicine,Chinese Medicine Prescription Dictionary,Chinese Modern Chinese Medicine Periodical Collection about the external treatment of Chinese medicine before September 30,1949 in vertigo literature.According to the collection result of literature,the Chinese medicine external treatment to treat vertigo development origin was clear.The first part,this research uses computer retrieval combined with manual retrieval of Dictionary of Traditional Chinese Medicine,Chinese Medicine Prescription Dictionary,Chinese Modern Chinese Medicine Periodical Collection,searched with "vertigo" as the subject term,and searched according to the established search strategy.The literature on the treatment of vertigo is from the pre-Qin and Han dynasties to September 30,1949.The establishment of a literature database of external treatment of vertigo,through in-depth analysis of the literature on the treatment of vertigo in various periods before the founding of the People's Republic of China,and perfect the development of external treatment of vertigo.b.The rule of acupoints and external medicine study was divided into two part.Literature before the founding of the People's Republic of China,to establish the database due to the result of the first study.Literature after the founding of literature retrieval in CNKI,VIP Database,Wanfang Database,Sinomed Database,from October 1,1949 to September 30,2020,all external treatment of TCM literature for vertigo,establish database of external treatment for vertigo of ancient and modern literature,external use medicine database,acupoints database.With the ancient and modern basis cloud platform,IBM SPSS Modeler18.0 software,frequency analysis,clustering analysis and association rules analysis were applied in acupoints,external use drugs.c.Randomized controlled clinical Trials of TCM external treatment for vertigo were retrieved and the effectiveness and safety of TCM external treatment for posterior circulation ischemic vertigo were systematically evaluated.The Randomized controlled clinical Trials were searched from CNKI,VIP database,Wanfang database,VIP Database,Sinomed Database,Cocharane Library database,Embase database,Web of Science database and Science Direct database.The starting and ending time of literature retrieval was from the establishment of the database to September 30,2020.Results: a.68487 literature were searched in Tencyclopaedia of Traditonal Chinese medicine,2214 literature into the original database,1043 pieces of literature into the data mining statistical database,involving 26 external treatment methods,including acupuncture,moxibustion,Gua Sha,and herbal pillows,etc.b.High frequency acupoints in acupuncture are Feng Chi(GB20),Nao Kong(GB19),Tou Linqi(GB15),Hou Ding(GV19),Jie Xi(ST41),Tian Zhu(BL10).The commonly used acupoints couples are Jie Xi(ST41)and Feng Long(ST40),Tong Li(HT5)and Fei Yang(BL58),Gan Shu(BL18)and Shen Shu(BL23);High frequency acupoints in moxibustion include Xin Hui(GV22),Tao Dao(GV13),Feng Chi(GB20),Qian Ding(GV21),Shen Ting(GV24)and Hou Ding(GV19).The commonly used acupoints couples are Zhang Men(LR13)and Ran Gu(KI2),Da Zhui(GV14)and Tao Dao(GV13).For external use medicine of TCM,high frequency drugs for Vinegar,Ligutici Rhizoma,Asari Radix et Rhizoma,Saposhnikoviae Radix,Chinese Chive,Coptidis Rhizoma,Menthae Haplocalycis Herba,Natrii Sulfas,Moschus.Study on literatures after the founding of the People's Republic of China involve 2452 papers,19 types of Traditional Chinese Medicine external treatment methods,High-frequency acupoints are concentrated on the head and the base of the skull,represented by15 acupoints including Feng Chi(GB20),Bai Hui(GV20),Jing Jiaji,Tian Zhu(BL10),Da Zhui(GV14),Feng Fu(GV16),Tai Yang(EX-HN4)and Yin Tang(EX-HN3).These acupoints are mainly to Du meridian,Gallbladder meridian,Bladder meridian,Stomach meridian,and ear acupoints and acupoints outside meridians are uaed frequently.External treatment of traditional Chinese medicine mainly includes medicine pillow,fumigation,external application,hot ironing,ion introduction,etc.High frequency herbs include Ligutici Rhizoma,Carthami Flos,Angelicae Sinensis Radix,Puerariae Lobatae Radix,Gastrodiae Rhizoma,Notopterygii Rhizoma et Radix,Salviae Miltiorrhizae Radix et Rhizoma,Clematidis Radix et Rhizoma,Olibanum,Cinnamomi Ramulus,Speranskia tuberculata,Myrrha,Angelicae Dahuricae Radix,Chrysanthemi Flos,Borneolum Syntheticum,Asari Radix et Rhizoma,Paeoniae Radix Rubra,etc.c.202 literatures were detected in the systematic review study for the first stage,and 53 RCT literatures were included,all of which were Chinese literatures,English literature was not adopted because it did not meet the inclusion criteria.Based on the systematic review and meta-analysis of 53 literatures,external treatment of Chinese medicine for posterior circulation ischemic vertigo can improve the symptoms of vertigo.The clinical effective rate was better than that of western medicine alone,and the difference was statistically significant.Conclusion:a.Chinese external treatment for vertigo originated from the Q in and Han dynasties,which has been uesd till today.b.In terms of acupoints selection,acupoints on the head and neck are the main acupoints.In termsof external treatment of TCM,crude drugs and powerful drugs with thick smell and strong properties are mostly used,as well as aromatic drugs that can circulate meridians and collaterals,and open orifices and penetrate the bones.c.According to the evidence-based medical research results,the external treatment of traditional Chinese medicine or combination of external treatment and conventional treatment of western medicine has a significant advantage in improving vertigo symptoms and improving clinical efficiency compared with western medicine alone.From the perspective of safety,there is no evidence that external treatment of traditional Chinese medicine has obvious adverse reactions,and external treatment of traditional Chinese medicine should be recommende in the clinical treatment of posterior circulation ischemic vertigo.
