| Method Collecting Chinese-English random contrast study literature which have and haven’t been reported until Oct,2012. on hypertensive vertigo with traditional Chinese medicine,according to evidence-based medical method,evaluate the efficacy and safety,consequently providing medical evidence on treating hypertensive vertigo with traditional Chinese medicine,and offer new thought on clinical deep research.Data and Method establish the strict inclusive and exclusive criteria,entirely retrieve Cochrane Library (2012,Issuel)、Medline or Pubmed (1980-2012). Ovid evidence-based medical database (Cochrane DSR, ACP Journal Club, DARE, and CCTr,1991-2012)、Embase disk database (1978-2012)、Chinese biology medical literature disk database (CBM disc)(1978-2012)、CNKI database(CNKI Web www.cnki.net)(1994-2012)、VIP Chinese technique magazine database (1959-2012)、Wan Fang resource database (1975-2012).obtained literature referring to inclusive criteria, will conduct quality valuation and data collection,finally conduct literature quality valuation and data compound on the inclusive study,Two independent valuers value the5sections:producing from random distribution project、distributing project hide、blind method、the integrity and selective report of final data.And it uses valid and invalid case numbers、 values the efficacy on hypertensive vertigo with traditional Chinese medicine.All the data was analyzed by RevMan5.2software produced by Cochrane,technique data,OR or Relative Risk and95%Confidence interval;measure data,weighted mean difference or standardized mean difference and95%Confidence interval.Result I retrieve the literature266pieces in all,choose133pieces which is not related to the research from reading subject and abstract,after reading the whole article,diagnose on hypertensive vertigo and efficacy standard corresponding 《Traditional Chinese Medicine Illness Diagnose Efficacy Standard》 or《Chinese New Medication Clinical Research Guide Principle》,and treating time is more than2weeks,wipe off the literature125pieces,finally bring into8inclusive criteria clinical trial (644patients) are assessed by the system.8trial methods are irregular,mainly represent in randomly distribution program imperfection;lack of proper using blind method trial;not doing samples’estimate;not explaining the situation of leave and exit,not doing intentional analyze.After collecting data,do Meta analyze or descriptive analyze.The result shows OR is3.33;95%CI is2.15to5.17,Z is5.37, P=0.00001<0.05, point out the data is significantly difference,which means combined traditional Chinese medicine is more effective than only western medicine on treating hypertensive vertigo. Conclutions Clinical trial quality about traditional Chinese medicine treating hypertensive vertigo is generally low at present.with lots of methodology defect,especially not doing real random grouping and blind method evaluation.Although the data shows traditional Chinese medicine combined other therapy is more effective than only western medicine,the difference is not completely significant,and the study is lack of the support of a great quantity of clinical case observed result;the antihypertensive efficacy is not proportional to the improvement of vertigo symptom,after some patients’blood pressure up to the standard.the vertigo symptom haven’t improve obviously.furthermore the symptom aggravate with side-effect of the medicine.So we should discuss more effective and safe treating program,the next step,we should hold high quality、large sample RCT to clear the efficacy and safety of traditional Chinese medicine treating hypertensive vertigo,take the characteristic of traditional Chinese medicine into account,and extract the marrow of evidence-based medicine,explore and found Chinese evidence-based medicine which is fit into Chinese research,improve Chinese clinical research level to make material improvement,supply scientific evidence to clinical decision,finally discuss a new road to develop traditional Chinese medicine. |
