Part1Observation and comparison on the distribution of inflammatory cells in nasal polyps in different yearsObjective:To explore the histological changes, possible influence factors and relationship between recurred time and nasal polyps in8years by observing the distribution of inflammatory cells in nasal polyps, especially the change of number and infiltration of eosinophils.Methods:All the160patients with nasal polyps were selected from the hospitalized patients in Qingdao University Medical College Affiliated Hospital from2004to2011. Every20cases were picked out in each year and the age raged from19to79,122cases were male, the other38cases were female. All the patients were analyzed retrospectively and divided into4groups according to their clinical features. The selected cases were eliminated relevant diseases, such as asthma, nasal neoplasms, cystic fibrosis, primary ciliary dyskinesia, fungal rhinosinusitis and significant anatomy deformity. Besides, there were no antibiotics and corticosteroids used for one week before the operation.Each specimen of nasal polyps were stained with HE and counted the number of eosinophils, lymphocytes and neutrophils under a light microscope with high power field(×400). All the cases were divided into two groups according to the number of eosinophils:(1)eosinophilic infiltration and (2)non-eosinophilic infiltration. Calculate the mean of three kinds of inflammatory cells in different years, different clinical and histological types. The data was analyzed with One-Way ANOVA and t-test. Bivariate correlation test was used to analyzed the relativity between inflammatory cells and recurred time.Results:The difference of the mean number of eosinophils among8years has no significance(F=1.961, P>0.05), and the number shows an undulant trend year by year. The year2009has the highest eosinophil mean numbers following the year2011and2007. The difference of the mean number of lymphocytes and neutrophils was not significant(P>0.05). The difference of eosinophils number in different clinical types and histological types was both significant (P<0.01), but the same result didn’t show in the number of lymphocytes and neutrophils(P>0.05).In the two histological types, the eosinophils were found predominantly in the eosinophilic infiltration group, lymphocytes taking the second, neutrophils the least. But in the non-eosinophilic infiltration group, lymphocytes were found predominantly, following with eosinophils and neutrophils. The recurred time of patients in eosinophilic infiltration group is much longer than the other group(t=17.552, P<0.01), and the time was positive correlated with eosinophils number(r=0.255, P<0.01). The number of eosinophils and lymphocytes was negative correlated(r=-0.199, P<0.05), but the correlation of the number of neutrophils and eosinophils, lymphocytes was not significant(P>0.05).Conclusion:The number of eosinophils in nasal polyps presents an undulant trend in the late8years with no significant changes in lymphocytes and neutrophils. The year2009has the highest eosinophil mean numbers following the year2011and2007. There are different inflammatory cells infiltrating in nasal polyps in different clinical and histological types. The recurred time of patients in eosinophilic infiltration group was much longer than the other group. The more eosinophils they infiltrate, the longer recurred time patients will take. In a word, the classification of nasal polyps according to characteristics of inflammatory cells infiltration may be helpful to comprehend the mechanisms, guide the treatment and evaluate prognosis. Part2The significance on the expression of chemokine eotaxin and RANTES in nasal polyps in different yearsObjective:To explore the variation tendency and relativity between chemokine and nasal polyps in eight years by detecting the expression of eotaxin and RANTES.Methods:All the40patients with nasal polyps were selected from the hospitalized patients in Qingdao University Medical College Affiliated Hospital from2004to2011. Every5cases were picked out in each year and the age raged from21to78,28cases were male, the other12cases were female. Each specimen of nasal polyps were stained with HE and immunohistochemical staining, counted the number of eosinophils and measured the average optical density(AOD). All the cases were divided into two histological groups according to the number of eosinophils. Calculate the mean of three kind cells in different years. The data was analyzed with One-Way ANOVA、t-test and Bivariate correlation test.Results:The difference of the mean number and AOD among eosinophils, eotaxin and RANTES in8years were significant(P<0.05), and the number shows an increasing tendency year by year. The difference of the mean number and AOD among eosinophils, eotaxin and RANTES in the two histological groups were also significant (P<0.001). Besides, the number of eosinophils and eotaxin、RANTES were positive correlated (P<0.05).Conclusion:The number and AOD among eosinophils, eotaxin and RANTES in nasal polyps presents an increasing tendency in the late8years. There are different expression amout of chemokine in different histological groups. The expression amounts of eotaxin and RANTES in eosinophilic infiltration group are much more than the other group. The more amounts the eotaxin and RANTES express, the more eosinophils they infiltrate. This research may be helpful to comprehend the mechanism of eosinophils infiltration and guide the clinical therapy.
