Objective:In recent years, with the rapid development of modern medicine and the continuously improved treatment methods, mortality of acute heart diseases was obviously reduced。Combined with the extension of people’s average life span, the incidence of chronic heart failure rises year after year, and becomes the main clinical syndrome with high morbidity and mortality rate of the global. Studying the TCM syndrome characteristics of chronic heart failure has a positive significance to guide the clinical intervention and improve the curative effect of TCM treatment. This research retrospectively analysed the TCM syndrome characteristics of chronic heart failure and its relationship with gender, age, and other diseases combined,in order to further understanding the TCM syndrome characteristics of patients with chronic heart failure, thus can provide certain guidance for cl inical treatment of this disease with traditional Chinese medicine. Methods:This study includes 138 patients with chronic heart failure, and these cases are all come from the hospitalized patients in the Department of Cardiology in Dongzhimen hospital between February,2009 and January,2015. Among them,70 are male while 68 are female, age between 35 and 94 years old, the average age is 72.39 years old. All medical record datas was collected with case collection table, including patients’ basic information, TCM four diagnostic information and physical and chemical inspection, etc, establ ishing database with EXCEL2007 for data entry and using SPSS 20 statistical software for data analysis, using frequency statistics, bivariate correlation analysis and chi-square test to analyze. Results:In this study, the prevalence of chronic heart failure is from 35 to 94 years old, and the highest prevalence is between 61 and 80 years old. In the perspective of gender, the prevalence of male is higher than female under 60 years old, between 61 and 80 years old, no obvious difference was found in prevalence between male and female, however, the prevalence of female is higher than male above 80 years old.Among the 138 cases,99 combined with hypertension,97 combined with coronary heart disease, and 54 combined with diabetes, they together occupy the first three. Other diseases such as hyperlipidemia, cardiomyopathy, and chronic renal insufficiency, can also cause or aggravate chronic heart failure. In the analysis of syndromes, there are 93 blood stasis syndromes,78 Qi deficiency syndromes and 71 phlegm dampness syndromes, these three occupy the largest proportion, and accord with the pathogenesis characteristics of CHF. As to syndrome combination, there arc only 3 cases of single pattern, and the remaining 135 cases are all complex syndrome, among which throe syndromes combination is the most as 73 cases, that is to say 52.9 percent of all.In the study of the correlation between atrial fibrillation and blood stasis, blood stasis rate is 64.1% in 39 cases of atrial fibrillation group and 68.7% in the group of 99 cases of not atrial fibrillation, using chi-square test, the result shows that the chi square = 0.268, P = 0.688> 0.05,which has no statistical difference, it suggests that we can’t consider that blood stasis is associated with atrial fibrillation. This study suggests that the proportions of blood stasis syndromes are the largest both in male and female.which are 71.4% and 63.2% respectively. Furthermore, Qi deficiency syndrome and phlegm-dampness syndrome occupy the second and third respectively.Dividing age into four groups, then analyse the TCM syndrome, we found that in the patients between 41 and 60 years old, phlegm-dampness syndrome is the most, while blood stasis syndrome is the most syndrome in other age group. The proportions of dampness-heat syndrome and retained fluid syndrome are relatively smaller.Whatever disease they combined with, the proportion of blood stasis syndromes is the largest, while Qi deficiency syndrome occupy the second. The proportion of phlegm dampness syndrome is less than 50% in hypertension and hyperlipidemia, but besides this, in patients’with every other disease, the proportion of phlegm dampness syndrome is more than 50%, but both occupy the third. In patients with hyperlipidemia, the proportion of Yang deficiency syndrome is slightly greater than Yin deficiency syndrome, the proportion of retained fluid syndrome is slightly greater than dampness-heat syndrome, which is different from the patients with hypertension, coronary heart disease and diabetes. However, the difference is so small that has little clinical significance, and overall, through chi-square test, we found that there is no significant difference among the syndromes of patients with CHF, which combined with coronary heart disease, hypertension, hyperlipidemia and diabetes. Conclusion:The ago span is large in patients suffering from chronic heart failure, and most of the patients are elderly. The prevalence of female gradually increased as age grows. The three main background diseases of CHF are Hypertension, Coronary heart disease and Diabetes. The TCM syndrome of CHF is complicated,Qi deficiency syndrome, blood stasis syndrome and phlegm-dampness syndrome are the three main syndrome, which together compose the core pathogenesis. The most saydrome are complexed syndromes. There is no signifieant difference among the syndromes in age, gender and background diseasesPhlegm-dampness syndrome is very common in Middle-aged pati ents, while retained fluid syndrome and dampness-heat syndrome are infrequent in CHF.It has no rel ationship between Atrial fibrillation and blood stasis syndrome.
