ABSTRACTBackground:With the development of biology, mobilization, detection, collection and low temperature conservation technology of the peripheral blood stem cell (PBSC), especially the wide use of haematopoietic growth factors in APBSC mobilization and hematopoietic reconstitution, autologous peripheral blood stem cell transplantation (APBSCT) has been applied more widely in the treatment of acute leukemia , lymphoma and some solid tumors.Some studies have documented that the hematopoietic recovery is more rapid in stem cell transplantation (S CT) with haematopoietic growth factor-mobilized PBSC than with bone marrow. PBSC may be mobilized by chemotherapy alone, haematopoietic growth factors alone, or by chemotherapy plus haematopoietic growth factors. Some disease-specific chemotherapeutic agents plus haematopoietic growth factors may be the best choice at present.Objective:1 To assess the effect of no considering body weight and single-dose rhO-C SF(lenograstim, 250 micrograms) in combination with chemotherapy in APBSC mobilization and hematopoietic reconstitution of post-transplantation.2 To analysis the factors that influencing the yield of stem cells and hematopoieitc reconstitution,to estimate the the timing of HSC apheresis and to define minimum and optimum target stem cell threshold for APBPCT.3 To quest a applicable mobilization and transplantation regimen.Material and Method: Twenty-eight patients-18 with acute leukemia(AL),i0 with non-Hodgkin's lymphoma(NHL) -were enrolled in the study. All patients were given intensive chemotherapy with EA (Ara-C22gIm2Idx3d, VP-16 0.2-0.3gIm2Idx3d, 24 cases) or Cy(Cy 2-4gIm , 4 cases)regimens. rhG-CSF 250 micrograms/d subcutaneously was administered at the nadir of WBC count till hematopoietic stem cell separation . A rapidly rebounding white blood cell count from nadir was used to predict the time of peak PBSC release and to plan collection. Hematopoietic stem cell separation was performed in all patients by CS3000 plus blood cell separator when the WBC count was > 4.0 x l09/L or peripheral blood CD34~ cell percentage>1%. The conditioning regimen of transplantation was TBI+Cy or MACC.APBSC were infused on day 0. rhG-CSF was given from day +3.5-after stem cell infusion to the time of WBC count> 1.0 x 1 091L or neutrophil count >0.5 x iO9IL.Results:1 The nadir of WBC count was on the day 4 to 15 after chemotherapy. rhG-CSF used to mobilize PBSC had been administered for a median of 6 days (range, 4-11 days). The average dosage of G-CSF were 4.23 U g /kg/d (range, 3.2 1-5.56 Li g Ikgld). 22 cases were performed one harvest and 6 cases had two harvests. A median of 4.0 x 108 mononuclear cells/kg (range, 2.1-8.57 x 108 /kg), 6.9 x 106 CD34~ceIls/kg (range, 1.8-88.97 x 106 1kg) and 3.6 x io~ CFU-GMIkg (range, 0.2-28.9 x ~ 1kg) for each patient were obtained. All patients were transplantation by APBSC , the median times of an absolute neutrophil count of more than 0.5 x i09/L or WBC count of more than 1.0 x i&/L were 10 days (range, 7-16 days), blood platelets count of more than 20 x i09/L were 16 days (range, 12-30 days), rapid and sustained hematopoietic engraftment occurred in all patients.2 The percentage of peripheral blood CD34~ cells and the WBC count can be used as a predictor for the time of peak PBSC release and the occasion of PBSC collection.3 We demonstrated that different chemotherapy regimens(used?in mobilization)and kind of disease were important factors influencing the yield of stem cells via the partial correlations procedure. Therefore, using these parameters, we found that the patients with non-Hodgkin抯 lymphoma may harvest more APBSC than that with myelocytic leukemia and acute lymphocytic leukemia. Effect of regimen of EA+G-CSF was better than that of Cy+G-CSF. The partial correlation revealed a significant correlation coefficient at the 0.05 level.4 We also found that the CD34~ cell was the strongest predictor of hematologic recovery after PBPC transplantation ,which was proved by Line...
