Objective To determine the dynamic change of the levels of peripheral blood PGE2, ll-2/sIl-2R and T-Iymphocyte subsets, and explore the relationship between PGE2 and IL-2/sIL-2R system in patients with colorectal carcinoma pre- and post-operation. To study the clinical significance of peripheral blood PGE2 and sIL-2R in patients with colorectal carcinoma. Methods From March,2000 to April 2001,61 patients with colorectal carcinoma were accepted in the department of general surgery in the First Affiliated Hospital of AnHui Medical University, 36 were male and 25 were female, the average was 56.5 ? 12.6 years. In the study, 15 case had simultaneously distant metastasis to the liver and (or) lungs and 46 patients did not show any obvious distant metastasis. In the former group, 9 patients underwent palliative resection and 6 patients laparotomy. All of the patients in the latter group received resection. 30 people found healthy at physical examination were considered as controls. According to Dukes classification, the pathologic diagnoses placed 20 patients at stage B, 26 patients at stage C and 15 patients at stage D. The levels of peripheral blood PGE2 and 11-2 pre- and post- operation as well as the PGE2 of tumor tissues in 61 patients with colorectal carcinoma were assayed by radioinimunoassay 1251-RIIA system, the level of peripheral blood sIL-2R, CD4~ and CD8~ pre- and post-operation were assayed by enzyme-linked immunosorbent assay ( [LISA ) and 5 immunofluoresence methods respectively. Results: The patients with colorectal cancer had a high concentration of peripheral blood POE2 and sIL-2R before operation. The levels were considerably higher than the normal in this study. But the level of 11-2 and CD4~ICD8~ in peripheral blood were lower than the control group before surgical operation. The peripheral blood POE2 and slL-2R had a negative correlation with IL-2 and CD4~ICD8~. The peripheral blood POE2 and sIL-2R was higher in the patents at stage D than at stage B and C. The peripheral blood POE2 had a positive correlation with twnor size. The level of POE2 in tumor tissue was 3.7-fold as much as that in normal bowel mucosa, and considerably higher than the hepatic metastasis tumor and polyp tissues. The levels of sIL-2R and POE2 were lower after the original tumor was resected and the levels of IL-2 and CD4~/CD8~ were higher after operation than before operation, The level of POE2 and sIl-2R decreased almost to normal in the patients with radical resection, but were still higher in those with palliative resection. The levels of POE2 and sIL-2R increased again when the metastases or recurrences of tumor occurred in 6 patients after operation. Conclusion 1) The imfnunosuppression of the malignant tumor originated from the tumor which related with the tumor derived immunosuppressive factor from tumor cells. The necessity of removal of tumor may improve the systemic cellular immunity state of patients. It was essential to provide early supplementary treatment with suitable biotherapy after operation. 2)PGE2 and sIL-2R participate in the immunologic hypofunction existing in the patients with colorectal carcinoma before operation. It is feasible in theory that NSAID and exogeous 11-2 are applied to prevent the metastasis or recurrence of colorectal carcinoma. 3) The level of peripheral blood POE2 and sIL-2R may be considered as an auxiliary monitor maker of whether recurrence of distant metastasis has developed after redical res...
