The incidence rate of the disease of prostate.mainly benign prostatic hyperplasia(BPH)and prostate cancer(PCa) has increased in recent years.It is necessary to research the pathogenesis of the disease of prostate for effective therapy so as to improve the quality of life of the Old-Age men.Endothelin(ET) is the strongest polypeptide at present in vivo that can contract blood vessel.It play an important role in cell proliferation and apoptosis and also take part in occurrence and progression of BPH and PCa.Insulin-like growth factor(IGF) is a polypeptide whose functions are like insulin.lt can accelerate cell differentiation and proliferation and can produce effects in development of normal proatate (NP) and disorders of proatate.In this study, the expression of ET-1, endothelin A subtype receptor (ETAR) and insulin-like growth factor I receptor(IGF- I R) in the tissue of NP, BPH and PCa was investigated by using the immunohistochemistry and computer-aided image analysis. In the meantime,the tissue components of BPH and NP were compared.Result: 1. Positive staining of the ET-1 was found in NP,BPH andPCa. Positive staining was localized mostly in nuclei of glandular epithelium and stroma smooth muscle cells of benign prostate tissues.Positive-staining was expressed mostly in cytoplasm of glandular epithelium of malignant prostate tissues.The percentage of positive-staining nuclei in glandular epithelium cells were (31.3 + 4.5) %, (80.5?.7) % (p<0.01) and (16.1 + 7.6) % (p<0.01) inNP, BPH and PCa.respectively. The percentage of positive-staining nuclei in prostatic stroma were (24.59 + 7.37) % and (33.89 + 3.90) % (pO.Ol) in NP and BPH.respectively.The areas of positive-staining in glandular epithelium were (5.9?.5) %, (15.9?.9) % (p<0.0\) and (1.4?.9) % (;?<0.01) in NP.BPH and PCa, respectively.2. Positive staining of the ETAR was found in 3/7 cases of NP, among whom 2 cases were + ,1 case was ++. Positive-staining of the ETAR in BPH was found in 22/30 cases,among whom 4 cases were +, 10 cases were ++, 8 cases were +++ . Positive-staining of the ETAR was found in 12/15 cases of PCa, among whom 1 cases were + , 6 case was ++, 5 cases were +++ . Positive staining of ETAR was localized in cytoplasm of gland epithelial. There was negative staining of ETAR in stroma.The absolute valute of gray scale were 15.53?4.13. 35.88?3.81 (/?0.01) and 31.76 ?0.23 (PO.05) in gland epithelial cells of NP , BPH and PCa .respectively.3. Positive staining of the IGF- I R was found in NP,BPH and PCa and localized in gland epithelial cells.mainly in membrane and cytoplasm of basal cell.The absolute valute of gray scale were 18.13?.24, 36.58?4.11 (PO.01) and 30.78?13.61 (7><0.05) in gland epithelial cells of NP , BPH and PCa respectively.4. Positive staining of ET-1 * ETAR and IGF- I R was found in PC-3Mcells. Positive staining which to show even bead distributs localized in cytoplasm of cancer cells.5.The hyperplasia is mainly occurred in stroma tissue in BPH. Stroma/gland, stroma/epithelia and lumina/ epithelia ratios were (1.70 + 0.58) /I , (3.70+1.24) /I and (1.14 + 0.1) /I,respectively, which were significately increased in comparison with that of normal prostate (P<0.05./><0.01and P<0.05.respectively).The percentage of stroma areas were decreased, and the percentage of gland.epithelia and lumina areas were increased as the prostate volum was increased. The hyperplasia of stroma probably was the basis of the other tissues hyperplasia.The results demonstrate that the expression of ET-1, ETAR and IGF-I R has increased in the disease of prostate,which show that they have synergism in the pathogenesis of disease of pro state. Further study on the effects of antagonist of ETAR and IGF- I R on the pathophysiology of the disease of prostate can provide the theoretical and experimental basis for effective therapy for BPH and PCa.
