Colorectal cancer(CRC) is a common malignant tumor, whose morbility keeps rising in China and in other developed country. As metastasis is the most important death cause of CRC, it is very important to evaluate the metastasis potential exactly.Latest study show that the expressions of CD44v6,nm23-H1 and CEA are related to invasive and metastatic potential of malignant tumor. CD44v6 is a variant of glycoprotein CD44 containing exon 6. By binding hyaluronate, CD44v6 influence the binding of tumor cell and the extracellular matrix, and enhance the cellular mobility, which affect the invasion and metastasis of tumor cell at last. Nm23-H1 gene is regarded as a metastasis suppressor gene, and it's product has been shown to be human neocleoside diphosphate kinase (NDPK). The variation, missing or lower expression of nm23-Hl gene are related to metastatic potential of malignant tumor. Carcinoembryonic antigen (CEA)is the most commonly used biomarker of CRC. There is overexpression of CEA in CRC, which probably is the characteristic after malignant changing of tumor cells. There is increasing evidence for correlation between CEA and invasion and metastasis of CRC.Our experiment is conducted to study the expression of CD44v6,nm23-H1 and CEA both in CRC tissue and normal colon tissue by using immunohistological technique ,to understand the relation of the expressions with pathologic characteristics especially with metastasis and the relevance among the expression of the three., and to understand the role of these expressions in the occurrence, development and metastasis of CRC.Materials and MethodsA total of 76 colorectal cancer tissues with their normal mucosa were obtained from patients operated in Colorectal Surgical Department of the First Affiliated Hospital of Zhejiang University from March 2001 to November 2001. Among these 76 patients, 46 cases were males, 30 cases were femals, mean age was 58.9 years old, 25 cases suffered from colon cancer, 51 cases suffered from rectal cancer, 31 cases with lymph node metastasis, 13 cases with distant metastasis. CD44v6, nm23-Hl and CEA were stained immunohistochemistry (IHC) in 76 CRC tissues and 43 corresponding normal tissues, and the correlation of their expressions with clinic-pathological variables were evaluated.Results1. Expressions of CD44v6 and CEA protein in carcinoma tissues were higher than para-carcinoma tissue and normal colorectal mucosa (P<0.001). No significant difference of nm23-Hl protein expression was detected between CRC tissues and their corresponding normal mucosa (P>0.05) .2. Expressions of CD44v6 in CRC were associated with the lymph node involvement, Dukes stage and histological differentiation(P<0.05), but not associated with tumor location , gross type, histological type, invasion depth and distant metastasis,.3. Expression intensity of nm23-H1 in CRC was related to distant metastasis and Dukes stage(P<0.05). No significant relations were found between the expression intensity of nm23-H1 and tumor location, gross type, histological type, histological differentiation, invasion depth and lymph node involvement,4. Expression intensity of CEA in CRC was positively associated with lymph node involvement, distant metastasis and invasion depth(P<0.05), but not associated with tumor location, gross type, histological type, histological differentiation and Dukes stage.5. Analysis of CD44v6, nrn23-H1 and CEA combinatively can improve the sensitivity and specificity to evaluate the metastatic potential of CRC.6. There was no relevance between the expression of CD44v6 and nm23-H1 orCEA in CRC(P>0.05).Conclusion1.There are much higher expression of CD44v6 and CEA in CRC tissue compare withcorresponding normal tissue.2..The expression of CD44v6, nm23-H1 and CEA are related to metastasis of CRCpositively. Analysis of co-expression of CD44v6, nm23-H1 and CEA obviously raisedthe sensitivity and specificity to detect metastasis of CRC.3.There is no relevance between the expression of CD44v6...
