Background: Insulin resistance (IR) is the basic characteristic of type 2 diabetes mellitus . Free fatty acid (FFA) and tumor necrosis factor TNF-a play very important role in IR. TCM syndromes differentiation is the marrow of TCM.Aim: To study the characteristic and superiority of Traditional Chinese Medicine, and then help the clinic treatment of type 2 diabetes and metabolite-connecting disease.Material and Methods: According to WHO diagnosis criteria and TCMdifferentiation criteria, we selected 92 type 2 diabetes cases, and divided them into twoteams of Metformin and TCM differentiation. 30 cases for Metformin (16 male and 14female), the ages are from 40-69 years old, the average age is 51.82 9.21 years old,the process of disease is 3 months-6 years, the average is 3.21 1.47 years, the averageweight index is 24.32 0.42,15 cases of Yin deficiency and heat excess syndrome(YDHES, 15), 15 cases of Qi and Yin deficiency syndrome (QYDA, 15). 62 cases forTCM differentiation criteria (32 male and 30 female), the ages are from 40-69 yearsold, the average age is 50.45 9.91 years, the process of disease is 2 months-6 years,the average is 2.97 1.61 years, the average weight index is 24.56 0.58, 30 cases ofYin deficiency and heat excess syndrome (YDHES, 30), 32 cases of Qi and Yin deficiencysyndrome (QYDA, 32). Of the 62 cases, 16 patients belonged to phlegm and turbid stagnation syndrome, 26 patients belonged to heat-blood stagnation syndrome, 35 patients belonged to syndrome without heat-blood and phlegm and turbid stagnation. 30 healthy people (16 male and 14 female) were taken as the control group, the ages are 40-69 years old, the average age is 51.77 9.87 years old, the average weight index is 24.48 0.54We detected fasting plasma glucose (FPG) by glucose oxidase, fasting insulin (FIns) by chemiluminescent immunoassay, triglyceride (TG) and high density lipoprotein-cholesterol (HDL-C) by zymology, tumor necrosis factor-a (TNF- ) by enzyme linked immunosorbent assay (ELISA), the reverse of the product of FPG and Fins is used as the insulin sensitivity index (ISI), that is , ISI= 1/FPG x Fins.RESULT: 1. Compared with the control group, the IAI of type 2 diabetes was significantly lower (P<0.01), the level of TG was higher(P<0.01), the level of HDL-C was lower (P<0.01), the level of TNF-a was higher (P<0.01). 2.The IAI of type 2 diabetes of TCM syndromes were lower than that of the control group (P<0.01), the degree of IR was QYDS>YDHES, and the level of TNF-a, TG were higher than that of the control group (P<0.05, P<0.01); the differences between the groups (P<0.01) is clear, the level of HDL-C was lower in QYDS(P<0.01). 3. Compared with syndrome without heat-blood and phlegm and turbid stagnation, in heat-blood stagnation syndrome, phlegm and turbid stagnation syndrome, IAI is lower (P<0.05), TNF-a is higher (P<0.05), in phlegm and turbid stagnation syndrome, IAI is lower (P<0.05),TNF-a is higher ( P<0.01), TG is higher (P<0.01); compared with heat-blood stagnation syndrome, in phlegm and turbid stagnation syndrome, TG is higher (P<0.01). 4. Compared with the team of Metformin,after the treatment, in the team of TCM, the IAI (P<0.01) is similar; TG, TNF-a is much lower (P<0.01); the effect of reducing the TG is not as good as the Metformin. 5. After the treatment, in Yin deficiency and heat excess syndrome, IAI is much higher (P<0.01),in Qi and Yin deficiency syndrome, IAI is higher (P<0.05), ; in Qi and Yin deficiency syndrome, TG and TNF-a are much lower (P<0.01). 6. After treatmentjn heat-blood stagnationsyndrome, phlegm and turbid stagnation syndrome, IAI has no much change(P>0.05); in syndrome without heat-blood and phlegm and turbid stagnation, IAI improved much(P<0.05); in these three teams, TG is much lower than before(P<0.01); in phlegm and turbid stagnation syndrome, HDL-C is higher (PO.05), in syndrome without heat-blood and phlegm and turbid stagnation, HDL-C is much higher (PO.01), and both TNF-a is lower (P<0.01).
