Host immune function inhibition is one of the main causes of tumour development and failure of therapy .Tumour cells can excape from the surveillance of immune system by many ways invading into other organs to induce neoplasm metastasis. Tumour cells can produce many immune inhibitiors to anaesthetize the immune function and accelerate tumour growth. Dendritic cells are the most powerful antigen presenting cells presently. The extent of dendritic cells infiltration is useful to estimate the local immune response in various types of cancer. Dendritic cells of tumour local sites reflect the host anti-tumour immune response more exactly than that of peripheral blood.The HLA-DR antigen is a membrance-bound glycoprotein encoded by genes located in the HLA-D region of the major histocompatibility complex. It plays an important role in the regulation of the immune response. Proliferating cell nuclear antigen is an essential protein for cell-cycle regulation correlating with the growth potential. The clinical usefulness of PCNA for predicting prognosis of patient with cancer has been reportedfrequently.In order toevaluate the roles of S-100protein positive dendritiv cell,HLA-DR and PCNA in carcinogenesis and progression of gastric carcinoma as well as their interaction,immunohistochemical technique is applied in examining the expression of HLA-DR, PCNA and the infiltration of S-100 protein positive dendritic cell. It is expected that the study will help us to judge the prognosis ofgastric carcinoma and provide theoretical foundation for clinical immunotherapy.Materials and Methods: The infiltration of S-100 protein positive dendritic cell ,the expression of HLA-DR antigen and PCNA in 43 cases of gastric carcinoma tissues,24 cases of precancerous lesions and 10 cases of normal gastric tissues were examined by use of S-P immunohistochemistry. SPSS 10.0 was used to evaluate the statistical significance. Results:1 . The number of S-100+DC of precancerous lesions (29.67±12.33) and gastric carcinoma tissues (16.69±8.68) was significantly higher than that of normal gastric tissues (8.85±4.75) (P<0.01). The number of S-100+DC of gastric carcinoma tissues was significantly lower than that of precancerous lesions (P<0.01).2. In normal gastric tissues,precancerous lesions and gastric carcinoma, the positive rates of HLA-DR were 20.0%(2/10), 70.8%( 17/24) and 60.5%(26/43) respectively.There were significant differences (P<0.05) when precacerous lesions and gastric carcinoma compared with normal gastric tissues separately.3. The positive rates of PCNA in gastric carcinoma 74.4%(32/43) were significantly higher than that of normal gastric tissues 10.0%(1/10) and precancerous lesions 33.3%(8/24) (P<0.05).There were no significant differences (P<0.05 ) when the other groups compared with each other.4. S-100+DC of gastric carcinoma was related to the extent of differentiation,lymph node and distant metastasis. S-100+DC with poor-differentiated (14.46±7.49) was significantly lower than that with well-differentiated (20.09±9.46) (P<0.05). S-100+DC with lymph node metastasis (13.73±6.52) or distant metastasis (11.13±5.47) were significantly lower than that without lymph node metastasis (20.42±9.75) or distant metastasis (18.84±8.79) (P<0.05).The number of S-100+DC was nocorrelated with the patients'age,sex,the depth of invasion,TNM stage,tumor size. (P>0.05).5. The positive rates of HLA-DR were related to the extent of differentiation. The positive rates of HLA-DR with poor-differentiated 46.2%( 12/26) were significantly lower higher than that with well-differentiated 82.4%(14/17) (P<0.05).The expression of HLA-DR was not correlated with the patients'age,sex,the depth of invasion,TNM stage,tumor size,lymph node and distant metastasis(P>0.05).6.
