BackgroundAcute renal failure (ARF) is a common and clinically critical disease. About 200 to500 thousand patients suffer from the ARF in our country annually. Although the advancement in dialytic technology has protective effect on the outcome of ARF, the cost of dialysis inflicts a tremendous financial burden on patients and dialysis maybe cause many complications. So it is important to find economical and practical pharmacotherapy for ARF. Acute tubular necrosis(ATN) resulting from acute ischemia is the common form of ARF, and the ischemia/reperfusion(I/R) injury plays an important role in ARF caused by other reasons. Recent studies suggested that Astragalus injection has been demonstrated to be a certain protective effect to the renal, heart, cerebral I/R injury by measuring SOD, MDA, NO and ET-1 which are correlated with I/R injury. Some studies showed that cytokines played important roles in ischemia/reperfusion(I/R) injury. Modulating their production and reconstructing renal recovery mechanism maybe bring a new approach to the therapy of ARF. For example, tumor necrosis factor alpha (TNF-a) is one of the most important cytokines which take part in the renal ischemia/reperfusion injury, it is confirmed that interleukin-6 is an important cytokine in the pathogenesis and progression of immune kidney disease.Astragalus injection has been demonstrated to be a certain protective effect to the heart, cerebral I/R injury. Based on the previous studies, we applied Astragalus injection to the rats with acute ischemia/reperfusion(I/R) injury model which was made by placing the atraumatic hemostatic clamp in renal pedicles for 45 minutes. We hypothesized that Astragalus injection treatment may have the effect of alleviation the renal I/R injury by modulation the production of cytokines.Animal experimentObjective: To investigate the effect of Astragalus injection and the change and role of plasma tumor necrosis factor alpha (TNF-a), interleukin-6 (IL-6) and interleukin-10(IL-lO) in acute renal ischemia/reperfusion(I/R) injury.Methods: Rat renal I/R model was performed by placing the atraumatic hemostatic clamp in renal pedical for 45 minutes. Astragalus injection was used on the rat models. Plasma BUN, Cr, TNF-a, IL-6 and IL-10 were observed at various intervals. Results : 1.Changes of BUN and Cr: group I/R compared with group control, Plasma levels of BUN significantly increased from 24h to 96h post-reperfusion(p<0.01). Plasma levels of Cr increased earlier than BUN. Cr increased at 6h post-reperfusion (p<0.01).Group Astragalus injection treatment compared with group I/R, Plasma levels of BUN also increased, but was not obvious to group I/R, which was closed to normal at 72h post-reperfusion, whereas BUN was still increased at 96h post-reperfusion in group I/R. As far as Cr concerned, Cr was still increased in group Astragalus injection treatment and group I/R, but Cr of group Astragalus injection treatment was significantly lower than that of group I/R. 2, Changes of TNF-a: Group Astragalus injection treatment and group I/R compared with group control, plasma TNF-a Group Astragalus injection treatment had already increased since 3h post-reperfusion obviously, then gradually decreased and came to the normal level at 96h post-reperfusion. TNF-a of group Astragalus injection treatment was significantly lowerthan that of group I/R at 3h, 6h, 24h and 48h post-reperfusion respectively. 3. Changes of IL-6 and IL-10: group I/R compared with group control, IL-6 began to increase from 3h post-reperfusion significantly (p<0.01) , which was most obvious from 24h to 72h post-reperfusion and began to decrease at 96h post-reperfusion. IL-6 of group Astragalus injection treatment was lower than that of group I/R at 3h, 6h, 24h ,48h and 72h post-reperfusion. There was not obvious variation for IL-10 during rat renal I/R injury.Conclusion1. Astragalus injection treatment could relieve the degree of increases of BUN and Cr in the rats with acute renal ischemia/reperfusion(I/R) injury.2. Astragalu...
