IntroductionInvasion and metastasis as the key characteristics in malignant tumor catch the attention of many people. In this article we study the relation of MMP-9, ICAM-1, CD34 to invasion and metastasis of gastric cancer and their relationship.Materials and Methods54 gastric cancer specimens were selected from paraffin wax embedded specimens with full clinicopathological data. They are divided into different groups according to histological differentiation . 15 cases of normal gastric muco-sa were studied as control. Immunohistochemical staining for MMP-9, ICAM-1 and CD34 was performed with a standard avidin-biotin-peroxidase complex detection kit. Statistical significance was calculated by t-test,x2 test,relative analysis. The level of significance was set at P < 0. 05.ResultMMP-9 is high expression in gastric cancer and with the decrease of differentiation, lymphatic metastasis, long distance metastasis, increase of clinical stage, the expression is increase( P < 0.05).ICAM-1 is low expression in gastric cancer and with the decrease of differentiation, lymphatic metastasis, long distance metastasis, increase of clinical stage, the expression is decrease( P < 0.05).CD34 is high expression in gastric cancer and with the decrease of differen-tiation,long distance metastasis,increase of clinical stage, the expression is in-crease( P < 0.05 ).In gastric cancer the relativity of MMP-9 and CD34 expression is positive, r = 0.411 ,P =0. 002 <0. 01; the relativity of MMP-9 and ICAM-1 expression is negative, r = -0.406, P =0. 002 <0. 01; ICAM-1 and CD34 expression has no relativity, r = -0.097, .P =0.484.Discussion1. The test approve that in gastric cancer with the increase of malignancy the MMP-9 expression increase. The result suggest that through some mechanism the ECM is hydrolysis by the increased MMP-9, and this inaugurate a invasive pathway. The finding is that MMP-9 expression focus on the interface of cancer tissue and normal tissue. Cancer cell and normal cell are both high expression. So we can speculate that MMP-9 high expression is the result of inducement and promotion of cancer cell and host cell.2. The test approve that the CD34 expression increase with the increase of malignancy in gastric cancer. The result suggest that MVD increase can obviously promote the invasion and metastasis of gastric cancer. We can infer that the new microvessel is induced by gastric cancer cell. CD34 is an antigen that has close relativity with new, immature microvessel . In new, immature microvessel CD34 express highly, and promote microvessel grow. New microvessel structure is not perfect, they can not only increase the invasion area, but also promote cancer cell fall into blood and cause blood metastasis.According to the result we can believe that CD34 can become a new mark to diagnose, evaluate and examine gastric cancer.3. In vitro test approve that ICAM-1 is very important in tumor cell immunity. It can facilitate T cell to recognize tumor cell and active T cell. Our test show that gastric cancer tissue ICAM-1 expression obviously decrease ( P < 0. 05) , and with the malignancy increase the expression decrease (P < 0. 05). This suggests that tumor cell inhibit ICAM-1 expression through some mechanism. One possible situation is that tumor cell hypoxia inhibit ICAM-1 expres-sion; the other is that serotype sICAM-1 level raise competitive combine with receptor (LFA-1) and cause tumor cell escape from body immune system. Early gastric cancer ICAM-1 expression also obviously decrease, this change may take part in early stage of canceration.Cancer tissue ICAM-1 expression level can be regarded as a objective index to evaluate body immune state.4. Result analysis show that the relativity of CD34 and MMP-9 expression is positive, r = 0.411, P = 0.002 < 0.01, this suggests CD34 and MMP-9 have coordinate action and promote each other.
