Study On Rhubarb Combining With Shenmai Injection In Eld CHF Patients

Objective: Combined with TCM basic theory and modern medical advancement, this study considered that the basic pathomechanism of CHF was toxin accumlation together with deficiency of Qi and blood stasis. It pointed out a hypothesis that the injury of heart by toxin was an important pathomechanism of CHF, removing toxin is an important treatment among treating principles of strengthening the body resistance to eliminate pathogenic factors. Rhubarb(removing toxin and activating blood circulation to remove blood stasis) combined with shenmai injection should be superior to shenmai injection .Method: 195patients with CHF were enrolled in the study,they were divided into three groups at random: rhubarb combined with shenmai injection group(70 cases), shenmai injection group(62 cases),control group(63 cases). The blood samples were taken immediately before and after treatment from all the subjects.blood brain natriuretic peptide( BNP),TNFa ,CRP,arterial blood analysis and lactic acid and cardiac function were measured.Heart failure score is based on clinical features. Heart failure 40 rats were divided into 4 groups at random: Rhubarb group(10 cases), shenmai injection group(10 cases), rhubarb combined with shenmai injection group(10 cases), control group(10 cases).The rat siwmming time in water were measured.results: No significant difference in BNP,TNFa ,CRP,arterial blood analysis and lactic acid and cardiac function and heart failure score of three groups before treatment and significant difference in BNP,TNFa ,CRP,arterial blood analysis and lactic acid and cardiac function and heart failure score between the treatment groups and control group (P >0.05) .After therapy BNP,TNFa ,CRP,arterial blood analysis and lactic acid, cardiac function and heart failure score in the treatment groups were significantly improved that incontrol group(p<0.05). Most indexes manifested that rhubarb combined with shenmai injection group was superior to the shenmai injection group. The annimal experiment was done,either.Conclusion:It suggested that the treatment mechanism is related to reducing inflammatary factors(TNFa ,CRP,) and improving metabolism (arterial blood analysis and lactic acid) and cardiac function (BNP)of CHF and so on.This study primarily tested and verified the responsibility and availability of rhubarb combined with shenmai injection group in treating CHF,and reliable basis to choose good treatment method of integrated TCM and west medicine for CHF.
