Objective: Pregnancy induced hypertension (PIH) is one of the leading cause of maternal and fetal morbidity and mortality. The study on its etiology and pathogenesis which are still unclear remains one of important subjects of obstetrics. Recently, some investigations indicated that endothelial dysfunction and defective endovascular invasion of cytotrophoblast are two important links of PIH. Cellular adhesion molecules (CAMS) that mediate the adhesion of leukocytes to endothelial cells, play a key role in the cause of PIH, are essential to the successful pregnancy. We investigated the expressions of adhesion molecules on lymphocyte and monocyte in peripheral blood and the decidual tissue of PIH, in order to study the mechanism of PIH, further provide the experimental and theoretical basement to the prevention and therapy of PIH. Methods: The expressions of vascular cell adhesion molecule-1(VCAM-1), E-selectin, P-selectin and L-selectin in decidual tissues from 30 patients with PIH as trial group and 23 normal pregnancy women as control group, were detected by immunohistochemistry methods. The expressions of VCAM-1, L-selectin, integrinβ1, integrinα3 and integrinαV on lymphocyte and monocyte of peripheral blood were measured by flow cytometry.Results: 1. The comparison of adhesion molecules expre- ssion on decidua in two groups: VCAM-1 mainly expresses on the vascular endotheliocyte, the positive rates were 73.33% and 30.43% respectively for PIH and control group. The staining of P-selectin were localized on the membrane and in the plasm of vascular endotheliocytes, syntrophoblast cells and interstitial cells, its positive rates were 83.33% and 43.48% respectively for PIH and control group. Respectively compared with their control group, the expressions of VCAM-1 and P-selectin on decidual tissues in moderate and severe PIH were all significantly higher (P<0.01, P<0.05). There was no difference of their expressions between mild PIH and control group (P>0.05). E-selectin and L-selectin all express on the membrane and in the plasm of the vascular endotheliocyte, syntrophoblast cell, interstitial cell, but there were no differences on decidual tissues in PIH group and control group (P>0.05). 2. The comparison of adhesion molecules expression on peripheral lymphocyte from two groups: Compared with the control group, the expressions of integrinβ1 on peripheral lymphocyte in moderate and severe PIH were obviously higher (P<0.05); but the expression of mild PIH was not higher than that of control group (P>0.05); in the PIH group, the expression of integrinβ1 on lymphocyte in severe PIH was higher than that in mild PIH(P<0.05). Compared with the control group, the expression of integrinα3 on peripheral lymphocyte in severe PIH was obviously higher (P<0.001); but there were no differences among mild, moderate PIH and control group(P>0.05, P>0.05). The expre- ssions of integrinαV and L-selectin in moderate and severe PIH were respectively higher than those in control group (P<0.05, P<0.05); but there were no statistical significant differences between mild PIH and control group(P>0.05). There was no significantly different expression of VCAM-1 on peripheral lymphocyte between PIH group and control group (P>0.05). 3. The comparison of adhesion molecules expression on peripheral monocyte in two groups: Compared with control group, the expressions of integrinβ1 and VCAM-1 on peripheral monocyte in severe PIH were obviously higher (P<0.01, P<0.05); There were no significant differences of integrinβ1 and VCAM-1 among mild, moderate PIH and control group (P>0.05, P>0.05). The expression of integrinα3 on peripheral monocyte in severe and moderate PIH was obviously higher than that in control group(P<0.01); but the expression of integrinα3 in mild PIH was not higher than that in control group(P>0.05); Compared with control groups, the expressions of integrinαV and L-selectin on peripheral monocyte in PIH were not higher (P>0.05).
