Correlation Study Between The 16-slices CT Dynamic Enhancement And Microvessel Density,VEGF,MMP-2 Expression In Determining Invasion And Metastasis Of Laryngeal Carcinoma

Objective: To investigate the correlation between 16-slices spiral CT dynamic enhancement and tumor angiogenesis microvascular density(MVD),vascular endothelial growth factor (VEGF)and matrix metalloproteinase-2(MMP-2) in determining invasion and metastasis of laryngeal carcinoma from the angle of combining imaging and histopathology. Methods: Immunohistochemistry methods of SP were used to detect the expression of CD34,VEGF and MMP-2 in 40 cases laryngeal cancer tissue and all 40 cases patients underwent enhancement dynamic CT scan before operation.The enhancement imaging features PV,CER,D-T curve of laryngeal cancer lesions were studid correlatively with tumor MVD,VEGF and MMP-2 in different tumor site,T staging and with or without lymph node metastatasis. Results: Dynamic CT enhancement was excellent in detecting the extent of invasion and with or without cervical lymph node metastasis.Peak value(PV) of tumor lesions were 13 to 80.15HU,CER were 2.98% to 24.8%,PT were 1 to 2 miniutes.The PV,CER and MVD,VEGF expression were higher in T3+T4 staging laryngeal cancer than that of T1+T2 staging (p<0.05).The MVD and VEGF was higher in supraglottic cancer or with lymph node metastasis than that of glottic cancer or without lymph node metastasis.(p<0.05). The PV and CER of supraglottic cancer or with lymph node metastasis than that of glottic cancer or without lymph node metastasis,but no significant difference.(P>0.05).The patterns of time-density curve were correlation with tumor MVD,VEGF and reflected the distribution characteristics of tumor microvessels within laryngeal cancer lesions.MMP-2 was higher expressed in supraglottic cancer and T3+T4 staging than glottic cancer and T1+T2 staging(p<0.01).Though the group with lymph node metastasis was higher also than without lymph node metastasis,between them have no significant difference(P>0.05). Conclusion: 1. The microvessels is richful in laryngeal carcinoma,and the microvessels is abnormal.VEGF is a important angiogenesis factor,it can promote the form of neogensis mocrovessels,in favour of the invasion and metastasis.MVD and VEGF are the important marker for judging the invasion and metastasis of laryngeal cancer. 2. MMP-2 may be secreted mostly by laryngeal squamous cells,and its major fuction may be to accelerate cancer cell invade to surrounding tissue.It take important effects on the infiltration of laryngeal cancer.Their expression has the relationship with the advancement of laryngeal cancer. 3. CT dynamic enhancement is useful for evaluating the blood supply,tumor angiogenesis and reflect the distribution characteristics of tumor microvessels and the expression of MMP-2. To a certain degree,it reflects the invasion and metastasis of laryngeal carcinoma. And it is helpful for accessing the biological behaviors and prognosis of laryngeal carcinoma.
