Objective: To investigate the role of psychosocial factors, neuroendocrine and immunology in the episode of obsessive-compulsive disorder (OCD), and to explore if they have mediation role to symptom severity degree.Methods: 48 patients with OCD and 48 normal controls were involved in the study. The data of two groups have no significant difference by balance test. All the subjects themselves completed the following questionnaires: general condition questionnaire, MMPI, DSQ, LES, SSRS, SAS and SDS; Y-BOCS was evaluated by the researcher. ACTH, Plasm cortisol, blood T Lymphocytes, IL-2 and IL-6 were detected in 20 patients with the first episode of OCD and 20 controls.Results: (1) The T scores of F, Hs, D, Hy, Pd, Pt, Si, Pa were higher in OCD patients than controls; (2) The Middle and immature defense style scores were higher in OCD group than controls, but the mature defense style scores were lower in OCD patients; (3)The frequency and intensity of negative life events were higher in OCD group than controls; (4) OCD patients obtained more subjective social support than controls, but less utilization of social support; (5) The concentration of ACTH, Plasm cortisol was higher than normal controls; the level of ACTH have positive relationship with the score of obsession, SDS, D, Hy, Pt, immature defense style and the total score of Y-BOCS; the level of Plasm cortisol have positive relationship with the score of obsession, SAS, SDS, Pt, immature defense style and the total score of Y-BOCS; (6)Thesum of CD4+T Lymphocytes, the ratio of CD4V CD8+ and the level of IL-2 were higher in OCD patients than controls; (7) The sum of CD4+T Lymphocytes have negative relationship with the score of obsession, SAS and SDS in OCD patients, and the ratio of CD4V CDg+ have negative relationship with obsession and SAS, but the level of plasm IL-2 have positive relationship with of obsession, SAS and SDS;(8) ACTH were negatively related to CD4+T Lymphocytes and the ratio of CD4V CD8+, but positively related to CDg+T Lymphocytes and IL-2; Plasm cortisol were also negatively related to CD4+T lymphocytes and the ratio of CD4+/ CD8+, but positively related to IL-2.Conclusions: The personality, immature defense style, more negative life events and less utilization of social support in OCD patients are closely related to obsessive-compulsive symptoms; Psychosocial factors can influence endocrine system and immune system by neuroimmunoendocrine network in OCD patients.
