Expression Of α-SMA, MMP-2, VEGF-C, ER, PR In Breast Carcinoma And Their Prognosis Significance

Objective: The purpose of this study was to investigate the relationship among the expression of α-SMA, MMP-2, VEGF-C, ER, PR ,the clinicopathological characteristics in breast carcinoma and, especially in carcinomous mesenchyme, to elucidate their roles in tumor progression,metastasis and prognosis,in order to providing a new theoretical basis for clinical treatment and predicting prognosis. Methods: A total of 98 paraffin-embedded breast carcinoma specimens (60 cases with the full follow-up) and 18 non-carcinomatous breast tissues were stained with polyclonal antibodies and monoclonal antibodies using S-P method ,and the patients'prognosis significance were analyzed by involved statistic methods. Results: 1. The positive rates of α-SMA ,MMP-2 ,VEGF-C in 18 non-cancinomatous breast tissues were 22.22%(4/18), 11.11%(2/18), 16.67%(3/18), respectively. The positive rates of α-SMA, MMP-2, VEGF-C in 98 breast carcinomas were 90.81%(89/98), 64.21%(63/98), 84.69%(83/98), respectively. The difference between the two groups of patients was significant (P<0.01)。2.The comparison of the immunohistochemical staining results: There were significant difference of the expressions of α-SMA between different histological grades and between the pisitive and the negative lymphonode metastasis(P<0.01).The expression of α-SMA had no relatiomship with the other clinicopathological features (P>0.05). The expression of α-SMA was highly associated with that of MMP-2(rs=0.373,P<0.001),and had no relationship with the expressions of VEGF-C ,ER and PR(P>0.05). There were significant difference of the expressions of MMP-2 between different histological grades and between the positive and the negative lymphonode metastasis(P<0.05). There was an association between the expression of MMP-2 and the expression of VEGF-C(X2trend=4.017,P<0.05)and no association with the expression of ER , PR(P>0.05). There was an association with the ages of patients (P<0.05),and no relationship with the other clinicopathological features(P>0.05)and the expressions of ER and PR(P>0.05). There was no significant association between ER,PR,and the other clinicopathological features (P>0.05,two –tailed ).There was a close correlation betwecn ER and PR(rs=0.375,P<0.01). 3.The comparison of the clinicopathological features:In 40(66.67%) tumor size over ?2.0cm,17(44.74%)tumor size less than ?2.0cm,positive the lymph node metastasis were 1(33.38%) graede I, 21(48.84%) grades II, 35(67.31%) grade III, respectively. That indicated the lymph metastasis easily occurd in tumors which sizes were over ?2.0cm and in tumors with poor grades (P<0.05).The five-year survial rates were77.14%(27/35),96.00%(24/25) in the positive and the negative lymph metastisis respectively.The lymphonode metastasis had a correlation with the survival rates.The survial rate of lymphomode metastasis was lower than that of non-lymphonode metstasis(P<0.05). Conclusion:The expression levels ofα-SMA,MMp-2 and VEGF-C are much higher in the tumour than those in the non-carcinomatous breast tissues .The expression levels of α-SMA and MMP-2 are much higher in the group with positive lymphonode metastasis than those in the other group with negative lymph metastsis.Lymphonode metastasis status can be served as a clinicopathological marker of prognosis.By detecting the lymphonode metastasis status, α-SMA and MMP-2,we can provide some helpful evidence for the treatment and prognosis of the breast carcinoma in clinic practice.
