Objective The study intended to explore the main risk factors of esophageal carcinoma , the relationship of the genetic polymorphisms of CYP1A1 Msp Ⅰand SULT1A1 and susceptibility to esophageal carcinoma(EC),and the gene-gene or gene-environmental interaction ,so as to provide evidences for making the integrated preventive measures and revealing etiological facter's mechanisms for esophageal carcinoma in the high accident place. Methods A case-control study which included 163 cases of esophageal carcinoma and 166 controls,matched according to age,sex, collected from Heping Hospital and Heji Hospital of Changzhi in Shanxi Province.Cases were correctly diagnosed by histological pathology and endoscopy ,and did not have any else tumor ;controls had no tumor history and disease of esophagus.All cases were investigated related factors of digest systemic disease and drawed elbow'venous blood 3ml to detect metabolic enzyme genotype,using polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).Statistics analysis adopt SPSS11.5 and SAS8.0 statistical package. Statistical methods use χ~2-test and logistic multiple factor regression analysis . Result 1.Analysis of multiple variables logistic regression showed that liking to eat pickled vegetable and hard diet,smoking,fierce mind stimulation,and family history of malignant tumor were the main risk of EC in Changzhi area, OR(95%CI) were 1.442(0.985~2.111),1.690(1.024~2.791),1.208(1.056~1.382),2.674(1.436~4.978),2.432(1.270~4.659). Animal protein and high degree of culture were the protective factors of EC, OR(95%CI) were 0.532(0.335~0.843),0.689(0.531~0.894). 2.There were no significant association between CYP1A1 MspⅠ,SULT1A1 genotype distribution and age,sex,smoking,eating pickled vegetables;CYP1A1 Msp Ⅰhave three kind of genotype : mutation homozygote(A genotype) , mutation heterozygote ( B genotype ) , wild homozygote(C genotype) ;frequencies of them were 27.61%,46.01% and 26.38% in case group, 14.46%,40.36%,45.18% in control group respectively(χ~2=15.49,P=0.00). Allele frequencies of T and G were 49.39%,50.61% in case group, 65.36%,34.64% in control group respectively(χ~2=17.17,P=0.00).As for SULT1A1 genotype , frequencies of Arg/Arg,Arg/His,His/His were 66.87%,30.67%,2.46% in case group, 77.71%,19.28%,3.01% in control group respectively(χ~2=5.75,P=0.06);Allele frequencies of G and A were 82.21%,17.79% in case group, 86.83%,12.57% in control group respectively(χ~2=3.37,P>0.05). 3. MspⅠmutation genotype showed significant difference between two groups,ORB=1.85 (95%CI:1.12~3.04), ORC=3.18 (95%CI :1.71~5.93);the variant SULT1A1 genotype (638GA or AA) was associated with a signification increased risk for overall EC ,OR(95%CI) was 1.73(95%CI:1.06~2.82). 4. Interaction analytical results:there were synergistic action between eating pickled vegetables and CYP1A1 MspⅠpredisposing genotype on the risk of EC:API=0.27,S=1.71,RERI=1.77 .There were synergistic action between eating pickled vegetables and SULT1A1 predisposing genotype,too: API=0.42 ,S=2.09,RERI=2.15. Conclusion Lack of animal protein ,liking to eat hard diet and pickled vegetables,smoking,fierce mind stimulation,low degree of culture, family history of malignant tumor were the main risk of EC in Changzhi area. CYP1A1 MspⅠand SULT1A1 mutation genotype were predisposing genes to esophageal carcinoma. People who eat pickled vegetables with CYP1A1 MspⅠ,SULT1A1 predisposing genotype have higher risk on the risk of EC.
