At present , more than ten millions of paralysis patients of spine cord injury survive in the word , and among them more than one million is in our country. According to statistic report, the number increases more than ten thousands of fresh cases per year in United States, and the accident rate of spine cord injury is about eight hundreds cases per year only in Peking area of China. Spine cord injury has been a type of high frequent disease. It is difficult to cure and has high deformity ratio, which resulting in paraplegia , quadriplegia and heart-lung-brain failure. Many patients lose the ability of labor and living, and which bring to great suffering and long-term burden including life , mentality and finance for themselves and their family as well. Spine cord injury has been a social problem. Research of Spine cord injury isincluding to study the mechanism of injury, the mechanism of restore , drug therapy, surgical treatment and subsequent function healing treatments and etc, all above are sedulous attention of anyone country in the world and anyone estate in our society. Whether the spinal cord cells injuried can regenerate , how spinal cord function can restore in a great degree, what is the mechanism of restoration, which have always been the heated issues in the field of neurology and also a direction of spine cord injury study laid down by international spinal cord trust fundation.Using laminectomy to decompressing of spine cord injuried is the initial treatment for chronic spinal cord compressive injury, furthermore ,mamy measures are applied to improve the degree of restoration of injuried spine cord, such as megadose (30 mg/kg) methylprednisolone , neurotrophic factors, acupuncture, physical therapy, high pressure oxygen therapy , and ect. But there were not satisfactory curative effect for the spine secondary injury so far. Many clinical practices and studies have shown acupuncture could mitigate spine secondary injury , improve the degree of restoration of injuried spine cord, decrease the malfunction of sense , motor , bladderor rectum and other complications. Therefore it is an important treatment for spine cord injury .However study of the mechanism of acupuncture treatment for chronic spinal cord injury is still in the early stage today. More experiment proofs are looked forward to . On purpose, a rat model of posterior progressive thoracic spine cord compression was made , and they received posterior decompression operation of thoracic spine and eletro-acupuncture therapy at later duration. The effect is evaluated by combined behavioral score (CBS), routine pathology and immunohistochemistry coloration detecting the expression of the insulin-like growth factor-(IGF-1). Then we could grope for the mechanism of acupuncture treatment for chronic spinal cord injury through the experiment.Methods: Forty Wistar rats ,2 months old, 200±50g weight , were randomly allocated to a model group including 30 rats and a control group including 10 rats (group A). The model group received moderate chronic progressive compression of thoracic spine cord within 8 weeks, then they were randomly allocated to three groups including 10 rats each: group B .received a sustained compression ofthoracic spine cord; and group C received decompression of thoracic spine cord; group D received decompression of thoracic spine cord and eletro-acupuncture treatment at the later duration. Clinical neurological function was assessed with combined behavioral score (CBS). At 11 weeks after injuried, all rats were assessed with combined behavioral score again, then they were sacrificed. The compression sections of thoracic spine cord were observed in morphologic and detected the change of expression of insulin-like growth factor-1 by immunohistochemistry coloration.Results: (1) Roentgenogram approve the vertebral canal encroachment rate of rats was 30%-60% after 8 weeks in the model group . Each rat in the model group developed clinical signs of myelopathy while they are normal of nerve function evaluated by combined behavioral score before experiment. Combined behavioral score in group B> C and D groups showed a consistent increasing trend, each of them is a notable difference from group A. In 11 weeks, group D recovered well in nerve function and evidently decrease of combined behavioral score was significantly different from group C (KO. 01) . (2) Immunohistochemistry colorationshowed the results of expression of the insulin-like growth factors-1 as following:Group A , low level expression; nerve cell of the insulin-like growth factors-1 presented very few and their coloration intensity are most identical; Group B, positive coloration cell counted few and most exist in neuroglia cell of compression section; Group C, positive coloration cell counted more in compression section, and most exist in neuroglia cell too; Group D, positive coloration cell counted more than GroupB and Group C , and so that in coloration intensity, locating dot in the cytoplasm was more evidently than in any other group. The data of Group D was significantly different from group C or Group B (K0.01) .Conclusions : (1) Expression of insulin-like growth factor-1 was few in the long-term spine cord compression, but increasing after decompression of spine cord. (2) After decomoression and eletro-acupuncture treatment of chronic compressive injury spinal cord , positive coloration cell of insulin-like growth factor-1 increase evidently, and so in coloration intensity. It could relieve the secondary spinal cord injury and protect the neuron and promote behavioral function recovery of rat. (3) We suggest thatinsulin-like growth factor-1 take part in the treatment of electro-acupuncture. Electro-acupuncture facilitates the restore of the spinal cord injuried.
