Objective: To explore the expression of survivin mRNA in exfoliate cell in urine and in the carcinoma tissue of the patients with transitional cell carcinoma of bladder(TCC), and to see whether or not survivin mRNA's expression associate with the occurrence, development, differentiation and pathologic grading and clinical staging of TCC. Methods: Fresh urine of 30 patients with TCC and 18 healthy volunteers who voided the second urine in morning was detected the expression of survivin mRNA in the urine by semquantitative reverse transcription polymerase chain reaction (RT-PCR). Meanwhile, Carcinoma tissue of 30 patients with TCC and bladder mucosa of 3 patients of benign prostatic hyperplasia(BPH) and 2 patients of traumatic bladder rupture was detected the expression of survivin mRNA by the same method. Results:In the urine samples, the positive rate of the survivin mRNA expression in the exfoliate cell in urine were 100% in the patient group and 22.2% in the control group, there were significance between them (P<0.01).The gray degree ratios of the survivin mRNA belt to the P-actin mRNA belt varied accordingly to different pathologic grades and clinical stages.( grade I: 1.00 ± 0.18, grade II: 1.28±0.14, grade III: 1.70±0.23, without break through basement membrane: 1.01±0.17,superficial muscle invasived:1.26 ± 0.14, deep muscle invasived:1.70±0.23, and there were significance between them(P<0.01)). In the tissue samples, All the individualexpression of survivin mRNA in the patient group are postive, while all negative in control. The gray degree ratios of the survivin mRNA belt to the p-actin mRNA belt were 1.24 + 0.35, 0.10 + 0.24 respectively. The gray degree ratios of the survivin mRNA belt to the |3-actin mRNA belt varied accordingly to different pathologic stages and clinical grades.(Stage I: 0.95 + 0.21, Stage II: 1.25 + 0.16, Stage III: 1.52 + 0.10,continence differed significantly between every two groups(PO.Ol); without break through basement membrane: 1.00 + 0.26,superficial muscle invasived: 1.23 + 0.16, deep muscle invasived: 1.47+0.17, and there were significance between them(P<0.05)).Conclusion: Detecting survivin mRNA in the urine can be used in diagnosing TCC, judging the malignancy degree of TCC and the prognosis of TCC. Deecting survivin mRNA in the mucous membrane of urinary bladder can be used in diagnosing TCC, judging the malignancy degree of TCC and the prognosis of TCC.
