Purposes: Hypoxia inducible factor -1 alpha (HIF-la) is a kind of reacting material to tissue hypoxia. During pregnancy, placenta trophoblast cell secrete HIF-1αto fulfil the different need in different time. When Hypertensive disorder complicating pregnancy occurs, placenta is short of oxygen and blood, which stimulates the trophoblast cell secretes more HIF-1α to endure hypoxia to continue the pregnancy. Leptin is an energy metabolism accommodation factor. It changes within the pregnancy to modulate energy metabolism When hypertensive disorder complicating pregnancy takes place, the secretion of leptin is seriously affected by endocrine and immune system disorder, and plays a role in pathophysiology The purpose of this study was to find the relation between HIF-1αand leptin through detecting them in placenta and umbilical vein blood, and the effect on the diseaseMethods: In this study, sixty-five patients were selected in hospital at random They were divided into five groups referring to "Gynecology and Obstetrics" (the sixth edition) There were ten cases were included in normal group, fifteen in gestational hypertinsion, fifteen in mild pre-eclampsia, fifteen in severe pre-eclampsia, ten in eclampsia. After the infant delivered, 1 cm3 placenta tissues were takenimmediately from the blood vessel plenty areas, and 4 ml blood was collected from the umbilical vein. Then the placentas and newborns were weighed. Immunohistchemistry was used to measure HIF-loun placentaes, radioimmunoassay was used to determine leptin level in umbilical vein blood. The mean optical densities (MOD) of HTF-1αstained by DAB were analyzed by high pathological image analysis system (HPIAS). Statistical data were described as Mean±S. According to the statistical principles, data were compared with analysis of variance. a=0.05 was considered as significantly. Result:1. HIF-1αpositive expression rate was 24.5%,and mean optical densities was 120.50±10.76 in the normal group , the gestational hypertinsion group were 33.4% and 125.29±26.90, the mild pre-eclampsia group were 69.4% and 186.47±26.75 , the severe pre-eclamsia group were 79.3% and 218.13±32.82 , the eclampsia group were 80.6% and 215.60±26.70. There was no significant difference between eclampsia group and severe pre-eclampsia group (p>0.05), severe pre-eclampsia group was different to mild pre-eclampsia (p<0.05), but they all were higher than gestational hypertinsion group (p<0.05). There was no significant differences between gestational hypertinsion group and normal group (p>0.05).2. Eclampsia group (29.85±2.97 ng/ml), severe pre-eclampsia group (27.14±3.35ng/ml) and mild pre-eclampsia group (23.07±2.82ng/ml) were significantly higher than gestational hypertinsion group (18.50±2.03ng/ml) and normal group (17.12±1.55ng/ml). There was significant difference between eclampsia group and severe pre-eclampsia group (p<0.05), and severe pre-eclampsia group was significantly higher than mild pre-eclampsia (p<0.05), there was no significant difference between gestational hypertinsion group and normal group (p>0.05).3. The normal group placentas and newborns mean weight were 643.00±45.72g and 3485.00±371.97g, gestation hypertinsion group were 622.33±23.97g and 3331.33±240.47g, mild pre-eclampsia were 585.00±43.34g and 3170.00±304.02g,severe pre-eclampsia were 520.67±25.20g and 2733 33±293.78g, eclampsia were 512.00±28.50g and 2550.00±222.36g.There was no significant difference between normal group and gestational hypertinsion group, eclampsia group and severe pre-eclampsia group of placenta weight But there were significant differences between other each group of it. There was no significant difference between eclampsia group and pre-eclampsia group, but they were all lighter than other three groups.4. According to normal group, gestational hypertinsion group, mild pre-eclampsia group, severe pre-eclampsia group and eclampsia group ,the correlations between HIF-laand leptin were -0.224 (p>0.05), 0.665 (p<0.05), 0.672(p<0.05), 0 740 (p<0.05), 0.854 (p<0.05).There was a positive correlation between HIF-laand leptin in hypertension group, but it had no means in normal group The correlations between HIF-laand placenta were -0437 (p<0.05), 0.664(p<0.05 ). 0.639 (p<0.05) ,0.818 (p<0.05) ,0.789 (p<0.05) The correlation in normal group was negative, while in hypertension groups,it was positive. The correlations between HIF-laand newborn weight were 0.441 (p>0.05) ,0.713 (p<0.05) ,0.623 (p<0.05) ,0.652 (p<0.05) and 0.802 lp<0.05) .5 According to normal group, gestational hypertension group, mild pre-eclampsia group, severe pre-eclampsia group and eclampsia group, the correlations between leptin level and newborn weight were 0.633 (p>0.05), 0.627 (p<0.05) , 0.582 (p<0.05), 0.634(p<0.05) and 0.753 (p<0.05). There was a positive correlation between leptin level and newborn weight in hypertension groups, but it had no significant correlation in normal groupConclusion:1 The expression of HIF-laimproves with the development of the disease HlF-la improves the development of infant and the growth of placenta.2 Leptin does great to the growth of infant and the pathophysiology of hypertensive disorder complicating pregnancy3. The secretion of leptin in placenta is significantly affected by the synthesis ofHIF-la.
