Objective To improve the effect of operative treatment of acetabular fractures by analyzing the clinical experiences in the treatment of the fractures. Methods 48 cases of acetabular fractures treated operatively from September 2000 to September 2004 were reviewed.A combined examination of roentenogram,CT scanning and the three-dimensional imaging of the spiral CT helps make an accurate diagnosis of acetabular fractures. All the fractures were classified according to the Letournel-Judet standard. The kocher-langenbeck approach,ilioinguinal approach and combination approach were used according to different fracture types. Reconstruction plates and screws were used for fixation. Results All cases were followed up for average of 18 months (ranging from 4 months to 39 months). According to the USA orthopaedic institution′s standard,36 cases were excellent (75%) ,9 cases good (18.75%) ,3 cases fair (6.25%). Conclusions Clinical analysis of the cases showed that surgery was a better option when the patients condition was good so that the fracture could almost achieve anatomical reduction.it suggests that the treatment should base on the type of the fracture.In the treatment,good reduction,rigid internal fixation and early functional recovery are essential.Much care,however,should be given to the selection of the operation approach,prevention of complications such as damage to the sciatic nerve,heterotopic ossification,venous thrombus and traumatic arthritis.
