Objective: To discuss the method for management ofacetabular fractures and evaluate the corresponding results.Methods: it was to summarize the experience in treatment of59 cases of acetabular fractures treated operatively since October2003 to October 2005.All the cases were classified according tothe Letournel-Judet classification and AO classification.Kocher-Langenbeck approach, illioinguinal approach and anteriorcombined posterior approach were adopted for different fracturewith reconstruction plates and screws.Result:All the cases were followed up for average 31months.According to criteria of Matta radiographicry reduction,there were 38 cases with anatomic reduction,18 with satisfactoryreduction, 3 with unsatisfactory reduction. Base on the modifiedMerle d'Aubigne and Postel clinical grading system, 18 cases(30.5%) showed excellent results, 26 (44.1%) good, 8(13.6%) fair,7 (11.8%) poor. Avascular necrosis of femoral head was found in 6cases (10.2%), heterotopic ossification in 5 cases (8.5%) andtraumatic arthritis in 7 cases (11.8%). No death, infection ornonunion were found.Conclusion: Operation on time, reasonable surgicalapproaches and anatomic reduction as well as early functionalrehabilitation are essential to good treatment results of acetabularfractures.
