Objective:To analyze and approach mortality risk factor of off-pump coronary artery bypass graft surgery. To analyze and approach correlated factor of the conversion from OPCAB to ONCAB. Methods:We selected cases of off-pump coronary artery bypass surgery in cardiothoracic surgery department of Tianjin medical university general hospital during 2003 to April 2006, of which 5 deaths happened. Based ACC/AHA, we have 34 selected risk factors. Univariate analyses were performed to evaluate the associations between death and different selected risk factor, significant factors were included in a stepwise logisitic regression analysis. Using the same method, we analysis the causes of the conversion from OPCAB to ONCAB Results:In all selected risk factors, operated shorter than 3 weeks after AMI and time of mechanical ventilation were found to be risk factors of mortality, whose odds rations were 69.76 and 1.135 . In preoperative selected risk factors, heart failure, operated shorter than 3 weeks after AMI, chronic obstructive pulmonary diseases, stenosis of left main branch were found to be risk factors of mortality, whose odds rations were 26.93, 15.35, 5.70, 1.03. Some risk factors of conventional coronary artery bypass graft surgery, including age, gender, obesity, diabetes, hypertension, renal inadequacy, were excluded statistical.Of all selected effected factors of the conversion from OPCAB to ONCAB, operated shorter than 3 weeks after AMI, stenosis of left main branch, LVEF were found to be the effected factors, whose odds rations were 18.253, 1.029, 0.913. Conclusion:The decrease of the number of risk factors showed the good early results inOPCAB due to its minimal invasiveness. A larger scale multiple-center assessment will be required.Trie correlated factor of the conversion from OPCAB to ONCAB is multiplicity, operated shorter than 3 weeks after AMI, stenosis of left main branch, LVEF were found to be the effected factors.
