Objective:To study the expression and localization of extracellular matrix metalloproteinase inducer(EMMPRIN) in deciduas and chorionic villi of first trimester pregnancy. Investigated the mechanism of medical termination and the process of threatened abortion. Methods:By using immunohistochemical analysis we evaluated EMMPRIN in deciduas and chorionic villi samples collected from: normal women undergoing early pregnancies (n = 30);women with threatened abortion in first trimester pregnancy (n=30);women undergoing medical termination(initiated by oral administration of 150mg mifepristone followed 48 h later by the administration of oral 600 μg misoprostol until expulsion of the embryo) (n=30). Results:1. There were significant differences in the expression of EMMPRIN in chorionic villi collected from medical termination or threatened abortion when compared with those of normal early pregnancy. The EMMPRIN expression of medical termination group was significantly higher than normal early pregnancy group(P<0.0005), as well as those of threatened abortion group were significantly higher than normal early pregnancy group(P<0.001).2. Representative photomicrographs demonstrating the EMMPRIN positivestaining was obviously appeared mainly on membrane of cytotrophoblast, syncytiotrophoblast, cytotrophoblast column of the villi of all the samples. All of the samples, the immunolocalization of EMMPRIN in cytotrophoblast column of the villi was the strongest (P<0.05). There was no apparent difference in localization of EMMPRIN between cytotrophoblast and syncytiotrophoblast (P>0.05).3. The positive staining of EMMPRIN was evident in deciduas of all the samples, but there was no significantly difference between normal early pregnancy and threatened abortion(P>0.05);medical termination and normal early pregnancy (P>0.05). Conclusion:1. EMMPRIN appears on chorionic villi of first trimester pregnancy, it can induce the production of MMPs, so that degradate basement and ECM of endometrium, participate implanting of chorionic villi.2. The mechanism of terminational medicine or the process of threatened abortion can be presumed as: it can up-regulate the expression of EMMPRIN in the membrane of syncytiotrophoblast, especially in cytotrophoblast column of the villi by a certain or some kinds of action, so that up-regulate production of MMPs in deciduas and chorionic villi. All the steps result in the degeneration and shedding of deciduas and the denudation of embryo.3. Whether progestone has effect on the expression of EMMPRIN in deciduas is still unknown.
