Objective: To explore the infection states of Helicobacter pylori in upper gastrointestinal diseases patients in Lanzhou area;To explore the relationship between pathopoiesis factors VacA,CagA and upper gastrointestinal dieases in Lanzhou area.To study on adhesion SabA's gene from separate strains of Helicobacter pylori in Lanzhou area preliminarily.Methods: Collected the paraffin embedding pathological samples of upper gastrointestinal diseases from living examination of gastroscope or operation samples . Adopted Grams stain and Warthin-Starry silver stain to detect Helicobacter pylori infection states in stomach tissue.Collected the blood serum samples of upper gastrointestinal diseases .Adopted immunblotting to detect the antibody of CagA, VacA,UreA and UreB and analyzed the relationship between VacA,CagA and upper gastrointestinal diseases in Lanzhou area.Collected the samples of living examination stomach tissue by gastroscope.Isolated culture Helicobacter pylori in vitro and identified it by morphologic and biochemistry methods.Adopted PCR to amplificate SabA's gene and estimated its carry states of Helicobacter pylori in Lanzhou area.Results:1 .In the paraffin embedding stomach tissue,the detectable rate by Warthin-Starry silver stain and Grams stain of Helicobacter pylori were 85.15 % (86/101) and 87.13(88/101) separately.They had not statistics significance.(P>0.05).Combined the both results,the infection rate was 74.26%.2.In the paraffin embedding stomach tissue,the detectable rate of Helicobacterpylori in the gastric carcinoma(GC) patients was 91.67%.It was higher prominently than the detectable rate(42.86%) in the chronic superficial gastritis(CSG) patients(P<0.05).But it had not significant difference to gastric ulcer(GU) and chronic atrophic gastritis(CAG) patients(P>0.05).3.By tests of blood serum,VacA antibody positive rate was apparently higher than UreA and UreB antibody(P<0.05).But it had not significant difference to CagA antibody positive rate(P>0.05).4.By tests of blood serum,the infection rate of I type' Helicobacter pylori strain was apparently higher than II type and middle type Helicobacter pylori strain (PO.01).5.By tests of blood serum,the CagA antibody positive rates of peptic ulcer disease(PUD) group and gastric carcinoma(GC) group were apparently higher than chronic atrophic gastritis(CAG) group (P<0.05).But it had not significant difference between PUD group and GC group(P>0.05).The VaCA and UreA antibody positive rates all had not significant differences in PUD,GC and CAG groups(P>0.05).And UreB antibody positive rate of PUD group was apparently lower than GC group(P<0.05).6.By PCR amplification,the part separate strains of Helicobacter pylori existed the adhesion SabA's DNA strip at 242bp in Lanzhou area.Conclusions:l.The infection rate of Helicobacter pylori in upper gastrointestinal diseases patients was high in lanzhou area(74.26%).2.Grams stain had the same value to Warthin-Starry silver stain in estimating the infection stats of Helicobacter pylori in gastric mucosa tissue of upper gastrointestinal dieases.3. The relationship between the incidence of GC and the infection rate of Helicobacter pylori was very close in Lanzhou area.And Helicobacter pylori also occupied an important position in invasion of GU and CAG.4.The pathopoiesis factors of Helicobacter pylori which affected the patients inLanzhou, VacA and CagA were the most important.5.The Helicobacter pylori which affected the patients in Lanzhou, I type strains were the most important. So the high pathogenicity strains were priority.6.Among the dieases which Helicobacter pylori caused in Lanzhou,the relationship between CagA and PUD and GC were very close.VacA and UreA all had relativities to PUD ,CAG and GC.UreB had relativity to GC.7. The Helicobacter pylori of Lanzhou area can express truly SabA's gene.
