PurposeGlioma has the highest morbidity among intracranial tumors, invasive metastasis is the main reason which causes failure of treatment and death, and it is closely related to the changes of multiple genetic and molecular levels. When primary tumor has been diagnosed by clinicians, a big part of the patients have sub — clinical recessive tumor metastasis colonies, and they ll finally develop to masses which can be seen in anatomy. So research on mechanism of glioma's invasive metastasis has significant value to the diagnosis and treatment. This research contrasts the expression of CD and PTEN to the pathologic characteristic and grading of human glioma, evaluates relations between CD, PTEN and pathological grades of tumor, and discusses their effect on invasive metastasis of gilo-ma.MethodSpecimen: 46 cases human brain glioma specimen which were cut off by surgery in neurosurgery department China Medical University affiliated NO. 1 hospital from December 2003 to may 2005. Male 29 cases and female 17 cases ages are from 14 to 76 years old. All the cases didnt accept radio - or chemical -therapy before surgery. Specimens are fixed by 10% neutral formalin, routine paraffin imbedding, 4μm serial section, and HE staining confirmed diagnosis, grade Ⅰ 13 cases, grade Ⅱ15 cases, grade Ⅲ 10 cases, grade Ⅳ 8 cases.Main agent;PTEN, CD anti - human monoclonal antibody of mouse, SPstaining kit and DAB kit both bought from Beijing Zhongshang Company.Empirical method and result assessment;using SP immunohistochemical method to detect the expression information of CD and PTEN in human brain gli-oma. Immunohistochemical positive is defined as brown drops or uniform brown exist in nucleus or cytoplasm, count the positive cells'percent in each cut sheet, no positive cells marked with ( - ) , positive cells less than 10% ( + ) , positive cells 10% ~50% ( + + ) , positive cells more than 50% ( + + +).Statistical analysis;all data uses x2 test.ResultCD positive staining mainly locates in cytoplasm, brown fine grain like, sporadic weak - positive expresses in well differentiated group ( grade I — II ), expression rate is 50% (14/28) . However, expression is obviously enhanced in poorly differentiated group( grade III—IV) , expression rate is 83. 3% (15/18). There is significant deviation between two groups (P <0. 05). PTEN positive staining mainly locates in cytoplasm, 18 PTEN positive expression cases in 46cases of glioma (39. 1% ) , in well differentiated group( grade I — II ), there are 15 positive expression cases in 28 cases (53. 6% ) , and in poorly differentiated group ( grade HI—IV), there are 3 positive expression cases in 18 cases (16.7% ). There is significant deviation between two groups (P <0. 05).DiscussionGlioma is the most frequent intracranial malignant tumor. Invasion and infiltration of tumor cell is one characteristic of glioma, and is also the basic reason why clinical total resection is hard and easy relapsing of tumor. BM of glioma can not be degradated completely by matrix catabolic enzymes, forming insect - erode like changes but not breakage, so tumor cells of glioma can only invade along BM and can not metastasis. Recent year study indicates that CD can decompose protein in basal membrane, facilitate the growth of tumor cell in dis-tal separative areas. In this research, there are expressions of CD protein in glioma cells, and expression rate reaches 63.04% , which indicates CD takes part in invasion process of tumor. We can guess CD plays an important role in invasive metastasis of glioma through degradating a series of natural barriers. The higher the malignant level is, the stronger CD expresses. And this is consistent with the clinical manifestation which higher malignant level glioma has stronger infiltrating invasive ability and larger range. This research shows that there are expressions of PTEN protein in gliomas, and there is significant deviation between two groups( P <0. 05). high grade group has higher absence rate, which suggests that PTEN genetic mutation or absence plays an important role in occurrence and development of glioma, and is closely related to malignant differential degree. Otherwise, high grade group has higher CD expression rate, stronger invasion ability, and higher PTEN absence rate, suggests that PTEN may inhibit the degradation of extra cellular matrix joined by CD.ConclusionResults of this research demonstrate that expression of CD and PTEN has connection with glioma grades, suggest that two different kinds of genes have regulation, induction or information transferring effect. Detecting expression of CD and PTEN is a significant biological parameter to make criteria for glioma malignant grades. And it provides a new way to study pathogenesy, invasive metastasis, prognosis prediction and treatment from different genetic changes.
