ObjectiveAccompanying the elevation of antifungal agents resistance to the fungi year by year , severe fungal infection increasing under immunosuppressive state and the limited ranges of and troubling side effects of antifungal agents , the combination one of clinical therapeutic strategies. From the literature some trial conclusions that seems to be self - contradictory. For instance, the same combination of antifungal agents in vitro to various fungi , sometime to the same fungi, have different interactions. In the clinic cases reports, we can also find that the same cases.The microdilution method was applied to study in vitro interactions of seven antifungal drugs according to M27 - A method of NCCLS. 30 strains of yeasts were used for combination study. To evaluated the antifungal activity in vitro of 7 kinds of antifungal drugs by determine their minimus inhibited concentration (MIC). Studied the interaction between bifonazole and Triamcinolone Acetonide two drugs. We also investigate the antifungal effects of bifonazole compound preparation in the animal model, our purposes is to offer the proof for the clinical therapeutic strategies.MethodsMIC of 7 antifungal drugs - bifonazole, Triamcinolone Acetonide, Encon-azole, Clotrimazole, 5 — Fluorocytosine, Terbinafine, Amphotericin - B against30 clinical fungal was determined by use of broth microdilution method. Measured the MIC when bifonazole combined with Triamcinolone Acetonide ( 10:1) , caculated the Fractional inhibitory concentration ( FIC). Guinea - pig animal model of dermatophytosis was made. The efficacy of bifonazol/ Triamcinolone Acetonide compound preparation cream (5: 1, 10 : 1,20: 1) f pevisone cream and bifonazol cream were investigated.ResultsThe antifungal activity of bifonazole was very strong against Filamentous fungiand and certain activity against Candida. It's activity better than Encon-azole, Clotrimazole. No interaction was found in combination susceptibility test. After being administered bifonazol / Triamcinolone Acetonide compound cream , the scores of skin lesions caused by trichophyton rubrum and trichophyton men-tagrophytes are( 1. 13 ±0. 83 and 1. 50 ±0. 76) respectively. It obviously reduce the degree of the local skin lesion comparing with the scores of skin lesions of the control group ( 3. 75 ±0.46 and 3. 88 ±0. 35 ) ( P < 0. 01). Ratio of 10: 1 better than ratio 5: 1 ( P < 0. 01) and bifonazole alone ( P < 0. 05) , no significant difference with pevisone cream. Bifonazole compound preparation also possess significant antifungal effect.DiscussionFungal infection is mostly endogenous opportunistic infection and attack the one in whom the body resistance decreases. The risk factors include chronic underlying disease, innune deficiency, administration of more than three drugs, systemic steroid administration and invasive operation 30 strains of fungal are isolated from the inpatient patients. We fond that compound Bifonazole and Triamcinolone Acetonide is possible according to FIC value. Meanwhile guinea - pig animal model of dermatophytosis was made. The efficacy of bifonazol/ Triamcinolone Acetonide compound preparation cream ( 5 '■ 1, 10 : 1, 20 ? 1) , pevi-sone cream and bifonazol cream were investigated. Ratio of 10: 1 better than ratio 5: 1 ( P < 0. 01) and bifonazole alone ( P < 0. 05) , no significant difference with pevisone cream. Bifonazole compound preparation also possess significant antifungal effect, there were significant reduction in the geometric mean for it. So this study can give clinical therapeutic a great supports in theory and in animal model.ConclusionStrong antifungal activity was found in Bifonazol. No interaction was found in Bifonazol and Triamcinolone Acetonide. There is stronger antifungal activity and antip ruritus effect with bifonazole compound preparation. The best ratio of Bifonazole to Triamicinolone is 10: 1.
