| Objective To explore the clinical features and operation indication of peptic ulcer bleeding and the effect of surgical treatment for Peptic ulcer bleeding .Methods From 2002 to 2006, the clinical data of 22 cases with peptic ulcer bleeding treated by operation were analyzed retrospectively. Results Findings in the operation and pathologic diagnosis: gastric ulcer in 11 cases (50%); duodenal ulcer in 7 cases(33.9 %); Postbulbar duodenal ulcer in 1 case (4.5%); complexity ulcer in 3 cases(13.6%); among them, canceration in 2 cases (9.1%) Procedures: subtotal gastrectomy with Billroth 1 anastomosis in 6 cases(27.3%), subtotal gastrectomy with Billroth II anastomosis in 15 cases.(68.2%), totalgastrectomy with Roux-en-Y anastomosis in 1 case(4.5 %), among them, duodenal ulcer exclusion gastrectomy in 3 cases(13.6 % ) Overall, hemostasis and ulcer was cured after surgical treatment in 21cases(95.5 %), 1 case(4.5%) died. Conclusions Correct evaluation of patient's preoperative status, in time operation and reasonable operative procedures are effective measures to improve the cure rate, subtotal gastrectomy is the commonly used procedure which is both effective in hemostasis and curing ulcer, and Vagotomy is the same in the effect, but less postoperative complications. |
