Objective: It suggested that the effective preservation period for donor heart still remained to 6 hours. It may lead to the heart transplantation failure following long-time hypothermic preservation. Therefore, our objective was to assess the efficacy of several cardioplegic solutions for donor heart preservation by comparing HTK solution, K-H solution and a solution from our laboratory, FWM Modified solution (FWM-M).Methods: We randomly divided male Wistar rats (n=20) to 4 groups: FWM, HTK, UW and Control. In the control group, hearts were harvested and immediately mounted on the Langendorff apparatus and reperfused. In the other three groups, the heart was rapidly excised and immersed in 1 of 3 preservation solution (4℃). The aorta was cannulated within 1 minute, and the cardiac coronary vessels were rinsed with the same solution for 3 minutes at a constant perfusion pressure (100 cm H2O). We paid great attention to achieving strictly homogeneous initial conditions for all hearts before applying perfusion with various preservation solution. At the end of the storage period, the hearts were immediately perfused in Langendorff model. Some indices of myocardial function such as left ventricular developed pressure (LVDP), the derivative of LV pressure (max and min dp/dt, reflecting both systolic and diastolic function), heart rate (FDR) and coronary flow (CF) were measured every 5 minutes after a 10 minutes stabilization period. At the end of the reperfusion, the speciman of the hearts were taken to be observed the changes of myocardial ultrastructure through light and electron microscope.Results: Our results showed that every experimental group had a different degree of worse heart function index. Group HTK and K-H had a worse mean left ventricular developed pressure (LVDP),dp/dtmax,dp/dtmin compared with group FWM modified (P<0.01). Group HTK showed better coronary flow (CF) compared to group FWM modified (P<0.05),Group FWM modified showed remarkably decreased release ofevery kind myocardial enzymes compared to group HTK (P<0.01 andP<0.01, respectively).Conclusion: When preserved in different kind of profoundhypothermia cardioplegia solution for 8 hour, heart function willdecrease ; The effects of FWM solution for hypothermic preservation ofisolated rat heart are better than that of HTK solution.
