ObjectiveIn order to investigate the expression of proliferating cell nuclear antigen(PCNA)and anti-metastasis gene(nm23) in hypopharyngeal and laryngeal squamous cellcarcinomas and its clinical significance. We applied the immunohisto-chemistrymethod to detect the PCNA and nm23 expression at molecule level. It is sensitive andspecific in analysis and estimation of micrometasis, the clinical and histopathologicalfeatures of tumor.MethodThe clinical and histopathological features(sex, age, growth pattern, infiltration,differentiation, T stage and clinical typing) of seventy-eight patients who acceptedoperations for hypopharyngeal and laryngeal squamous cell carcinoma from 1999 to2003 were reviewed. The immunohisto-chemistry method was utilized to stain thesefaculties.Result1. The expression of PCNA has no correlate with sex, age and infiltration. But ithas no relationship between nm23 and these features, nm23 has no correlatewith infiltration, growth pattern,differentiation,T stage and clinical typingelse.2. The positive expression of PCNA tends to increase with the progress of tumorT-stage and pathologic grades. 3. The expression of PCNA has significantly correlate with growth pattern:PCNA has higher masculine rate in the infiltrating type and exelcosis type.4. PCNA has different expression in the different clinical type: The expression ofPCNA in the supraglottic cancer is higher than the glottic and subglottic cancer;It is higher in the hypopharyngeal carcinoma than the glottic and subglotticcancer; but it is has no significantly correlation between supraglottic cancer andhypopharyngeal carcinoma.5. The expression of PCNA was significantly higher in patients with lymphnodemetastasis than those without lymphnode metastasis, and the nm23 negativeexpression has a significant association with lymphnode metastasis.6. The result showed a significant association between the expression of PCNAand nm23 and the recurrence of tumor. Hypopharyngeal and laryngealsquamous cell carcinoma with PCNA positive and nm23 negative expression isprone to occur recurrence.ConclusionThe expression of PCNA and nm23 had a significant association with the clinicaland histopathological features of tumor. The results suggest that PCNA, nm23 playimportant roles in the progression of hypopharyngeal and laryngeal squamous cellcarcinoma and are possible prognostic discriminators in hypopharyngeal andlaryngeal squamous cell carcinoma.
