Objective: To investigate the traditional Chinese medicine Tangzhifangintervention on the early macroangiopathy of diabetes mellitus rats model.Methods: 1.60 male Wister rats were divided into two groups randomly,the modelgroup is 50 and contral group is 10. The model group was feed with fat milk bylavage for 4 weeks, to measure fasting serum glucose and insulin, calculate theinsulin sensitivity index, to decide if the animal model was built up successfully.Then the model group was injected with small dose (30mg/kg) of 2%streptozotocin. Five days later,FBG was measured and the rats which have thecharacter of polydipsia, urorrhagia, polyphagia and which FBG are large than16.7mmol/L is selectived into the next experiment. 2. 45 rats which FBG are largethan 16.7mmol/L and 10 rats of control group are brought into the test, the modelgroup of 45 rats was divided into five subgroup: model contral group, group, low doseTangzhifang group, middle dose Tangzhifang group and high dose Tangzhifanggroup,every group is 9 rats. Observate the general condition of rats including drinkand food,weight,urine volume. Measure the FBG, total cholesterin, triglyceride, lowdensity lipoprotein, high density lipoprotein, endothelin at the time of before therapy,4 weeks and 8 weeks therapy. The upper aorta thoracica were dyeing withhematoxylin- eosine, and their's pathology were observated after 8 week therapy. Atthe same time,the lower aorta thoracica were dyeing with ranium- plumb andobservated with electron microscope. The islands of Langerhans were dying withimmunohistochemistry to observate the alpha cells and beta cells.Results:1. Lavaged with fat milk 4 weeks later, the weight of rats increased notablely, insulinalso rised obviously (P<0.01), FBG haven't went up and ISI degraded, all the rats formodel group copied successfully. After be injected with STZ, two died and three'sFBG hadn't reach to 16.7mmol/L, the achievement ratio of DM model is 93.8%.2. Dianoguanil and different dose of Tangzhifang all can cut down the FBG of DMrats effectively (P<0.01), but all the rats therapied can't cut down the FBG to normalrange, compared to the normals, difference is notable (P<0.01). The effect of middledose of Tangzhifang is best than others, the FBG is only 0.1% higher thandianoguanil group after 8 weeks later.3. The blood fat level of different dose of Tangzhifang group have been improved.The TC,TG and LDL of middle and high dose of Tangzhifang group are dropped tonormal range, can also rise the HDL to normal, there are no statistical significanceontrast with contral group (P>0.05), dianoguanil and middle dose of Tangzhifanghave the quicker effect of the total.4. The different dose of Tangzhifang all have the effect to depress the endothelin, the middle and high dose of Tangzhifang have the better effect. There are notabledifferences between model group and middle and high dose group (P<0.01), however,all groups's endothelin are higher than contral group (P<0.01).5. The endotheliocyte of rats's aorta thoracalis with T2DM insulin resistance havedamged partly, lipide settlings and cell necrosis cohere to the surface of endodermis,the coat of elasticity became thinner and has fewer layer and more myocyte, smoothmuscle cells are more, vacuolar degeneration, lipid droplet and collagen fibersincreased. While all dose of Tangzhifang group can stop the damage to endothelialcells, lipide and necrosis deposit effectly, to prevent macroangiopathy, the middledose are better.6. All groups have no effect with alpha Cell of islet; but have notable effect to betaCell of islet, compare to model group, volume is bigger and grains are more. Theeffect of middle group is better than dianoguanil.Conclusion: Tangzhifang have the effect of supplementing qi and nourishing yinand activating blood circulation to dissipate blood stasis.It can not only decreass theblood glucose, but also improve the level of blood fat. The result of experimentindicate that Tangzhifang can decrease the level of endothelin in blood plasm.Tangzhifang can partly recovery the beta cells of islet and can recovery partly injuryof the great vessel. It can improve the metabolic disorder condition of rats with type 2diabetes mellitus, reduce the possibility of macroangiopathy of diabetes mellitus.There is a dose-symbiosis between Tangzhifang and T2DM.
