Objective: Observe the variation of level of plasma IL-6 and Hs-CRP in patients with various TCM syndrome types of T2DM, explore the relationship between inflammative cytokines and various TCM syndrome type. In order to elucidate its TCM pathologic mechanism from the viewpoint of hypoinflammation in T2DM, aim to search for an objective index to improve TCM syndrome typing, guiding and facilitating the prevention and treatment on T2DM by TCM. Methods: Case group: 88 cases of patients with T2DM were divided into three subgroups according to the standard of TCM syndrome differenciation: 29 cases were Yin deficiency and Heat excessiveness, 31 cases were deficiency of both Qi and Yin, 28 cases were deficiency of both Yin and Yang. Control group: 30 normal volunteer were recruited, who were matched with the patients in according to age, gender, ethnic, body weight, ratio of waist / buttock. General material data were collected, plasma level of IL-6 and hs-CRP tested, the difference between two groups and the difference between any two case subgroups were compared respectively. Results: The level of plasma IL-6 and hs-CRP in patients with T2DM were significantly higher than those in normal control group (P<0.0l). In three case subgroups, from higher to lower, the sequence according the levels of hs-CRP were as follows: Type of Yin deficiency and Heat excessiveness; Type of deficiency of both Qi and Yin; Type of deficiency of both Yin and Yang. And the sequence according the levels of IL-6 were as follows: Type of Yin deficiency and Heat excessiveness; Type of deficiency of both Yin and Yang; Type of deficiency of both Qi and Yin. Every difference between two syndrome types is statistically significant.(P<0.05). Conclusion: There is a higher level of both IL-6 and hs-CRP in all three syndrome types of patients with T2DM, compared with those of normal control group. It confirms that there is do a hypoinflammative reaction in the development of T2DM. Meanwhile, there is also a significant difference between any two syndrome types of subgroup with regard to the levels of IL-6 and hs-CRP respectively, suggest that IL-6 and hs-CRP can be used as a potent objective index to improve the syndrome typing and effective evaluation guided with the TCM theory of treatment by syndrome differentiation, as well as a potential target facilitating the prevention and diagnostic of T2DM.
