Background and objectives:Collagenase chemonucleolysis is one of the most elective methods for the treatment of lumbar intervertebral disc herniation(LIDH).China,there are so many patients with LIDH received the treatment of collagenasc chemonuclcolysis every year.Ozone is known as one of the most intense oxidants.For the last decade,ozone has been used in some western-European countries fortreatment of lumbar discal herniation,and good results were reported.From 2000, ozone also has been used in some native hospitals for treatment of lumbar discal herniation, and also good results were reported and were worth using widely.But no experimental and pathological liter- atures on it were found to combined injections of ozone and collagenase in the treatment of lumbar disc herniation after cosulting a lot of doc- ument.Method of the thesis is observing the high effective effect of combined in- jections of ozone and collagenase in the treatment of lumbar disc herniation through experimental and pathological literatures.Methods:32 rabbits(big white ear of Janpan)are divided into 4 parts at random.A group(N equal 8, injections of NS in the treatment of lumbar disc herniation);B group(N equal 8,injections of collagenase in the treatment of lumbar disc hemiation);C group(N equal 8,injections of ozone in the treatment of lumbar disc hcmiation);D group(N equal 8,combined injections of ozone and collagenase in the treatment of lumbar disc herniation).Before L4-6 of rabbits are done lumbar disc herniation,let their a certain NCV detected.At the same time,also detect NCV in the first,seventh,fifth,thirtith day of lumbar disc herniation.At fast kill all rabbits and make samples.Pulp of intervertebral disk L4-6 to the samples give a survey of general and electric mirror and light microscope.Homology gray plate,caudex dorsalis,neurodokon and musculi psoas major will be given a observe of general and electric mirror.Pulp of intervertebral disk and nerve root is the most important.Results:Monitoring of NCV:Comparing every phase of A group after injection to preoperation,the preoperative first,seventh,fifth day slower in evidence(P<0.01 ).After the thirtith day of operration,NCV is yet slow but not significant difference(P>0.05 );Comparing the first day of B group after injection to preoperation,NCV reduce significant difference(P<0.01 ) The seventh,fifth day it is fater significant difference(P<0.01 );the thirties day it recovers to preoperation(P>0.05 ); Observation of pathematology:In group A The intervertebral disc was not dissolved,but the collage tissue loosen and weakly stained,the nerve root was seriously injured.The intervertebral disc was dissolved,the chondrocytes were dege- nerate,and the nerve root was slightly injured in group B.Conclusion:Extensive stuties of injection of collagenase and ozone in vitro indicate that it is an effective methed in prodoucing dissolution of nucleus pulposus.It can make never root which is under average by nuclei gelatinosus of ocular proptosis recover quickly.Comparing injection of collagenase or ozone to injection of combination,it is first-class.Collagenase and ozone do not destry good neurodokon and disci intervertebrales.Collagenase can make conduction velocity of nervi ischiadicus of neurodokon faster but ozone is not so.Combined injection of collagenase and ozone is a minimally invasive and effectively method for treatment of lumbar disk herniation.It is worth promotion in clinic.
