【Background】In recent years,with the further development of searching for more cardiovascular markers,we have already accumulated a lot of clinical experiences,especially the B-Type Natriuretic Peptide (BNP)and the plasma Cardiac Troponin I(cTnI)are of the most focus.It is acknowledged to most people that BNP is the marker of heart failure and cTnI is the marker of acute myocardial infarction,but less research has been down on correlation of BNP with arrhythmia,especially with atrial fibrillation,and whether cTnI correlates with the injury of cardiac muscle in patients undergoing RFCA(radiofrequency Catheter ablation). Besides,the atrial fibrillation radiofrequency ablation as a new kind of treatment of AF,the high recurrence has been the supreme problem since it's application,so it's necessary to discuss the of predictive factors of recurrence of postoperation atrial fibrillation in patients undergoing circumferential pulmonary vein ablation.【Objective】To discuss Troponin I(cTnI)and plasma B-Type Natriuretic Peptide(BNP)levels in predicting the recurrence of postoperation Atrial Fibrillation(AF)in patients undergoing circumferential pulmonary vein ablation of atrial fibrillation with the guide of CARTO-MERGE.【Methods】To analyze retrospectively the clinical data of forty six patients with atrial fibrillation who consequently underwent radiofrequency catheter ablation from January 2007 to January 2008 in sir-run-run-shao hospital.30 patients are Paroxysmal AF and 16 patients are persistent AF,with average history of 5.11y,all the patients have no marked symptoms of heart failure and renal inadequacy.Measure the cTnI concentrations 12—24 hours before cardiac operation and postoperation instantly,6hours and 24hours.Measure the postoperative plasma BNP concentrations instantly.Then,according to the symptoms, 12-leads ECG and Holter ECG of the patients after 3 months,we divided all the patients to two groups--succeed group(A)and recurrence group (B).【Results】The cTnI concentrations of 12—24 hours before cardiac operation and postoperation instantly,6 hours and 24 hours have no obvious clinical discrimination:but the postoperative plasma BNP concentrations in the group A obviously less than group B,(P<0.01). 【Conclusions】In the patients with Paroxysmal AF and persistent AF who have symptoms,the much higher postoperation plasma B-Type Natriuretic Peptide(BNP)levels they have,their recurrence of AF will be higher relatively.Whlie the Troponin I(cTnI)levels diversication have no obvious value in predicting the reccurrence of AF.Therefore,via measuring the plasma BNP concentrations in postoperative patients,we could predict the probability of AF and other tachyarrhythmia,and take effective and timely steps to depress the reccurrence of AF like reinforce the medication therapy.
