Objective To study the changes of fasting plasma visfatin level in different plasma glucose level crowd,and its significance and relation with IR,T2DM and obesity.Methods 80 cases were selected from the diabetic patients and healthy subjects of the inpatient department and medical examination center in the second hospital of Lanzhou University from March to September in 2007.According to the WHO about diabetic diagnosis standards in 1999,above subjects were divided into three group of type 2 diabetes group 51cases,impaired glucose tolerance group 6 cases and normal glucose tolerance group 23 cases.According to the Asia-Pacific region about obese diagnosis criteria in 1999,which takes BMI =25 kg/m~2 as the criteria to diagnose obesity,above subjects were divided anew into obese group of 43 patients and non-obese group of 37 patients.Subjects were measured plasma glucose,serum insulin,total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol(LDL-C)through collection median cubital vein blood after overnight fasting.Subjects were measured height,weight, blood pressure and calculated insulin resistance index(HOMA-IR)=FPG×FINS/22.5, insulin secretion index(HOMA-β)=20×FINS/(FPG-3.5).Subjects were measured glycosylated hemoglobin(HbA1c)by collection finger blood and measured fasting plasma visfatin levels through ELISA and collected the mean amplitude of glycemic excursions through continuous glucose monitoring system.Results 1 The fasting plasma visfatin levels of type 2 diabetes groups and impaired glucose tolerance groups were significantly higher than that of normal glucose tolerance groups(P<0.01,P<0.05),the difference between plasma visfatin levels of impaired glucose tolerance groups and that of type 2 diabetes groups was not significant(P>0.05).2 The fasting plasma visfatin levels of obese and non-obese groups were not significantly different(P>0.05).3 The fasting plasma visfatin levels of duration>5 years of type 2 diabetes groups were significantly higher than that of duration =5 years of type 2 diabetes groups(P<0.01),the fasting plasma visfatin levels of type 2 diabetes with complication groups were higher than that of type 2 diabetes without complication groups(P<0.05).4 Linear correlation analysis showed that the fasting plasma visfatin levels of different plasma glucose level crowd and glycosylated hemoglobin(HbAlc),low density lipoprotein-cholesterol(LDL-C)were positively correlated(r=0.317 P<0.01,r=0.248 P<0.05).However,the correlation between the fasting plasma visfatin levels and insulin,C-peptide has not yet found,the fasting plasma visfatin levels of patients with type 2 diabetes and the mean amplitude of glycemic excursions were positively correlated(r=0.562,P<0.01).Multiple stepwise regression analysis showed that glycosylated hemoglobin (HbAlc),diastolic blood pressure(DBP),waist-hip ratio(WHR)were the independent of relevant factors of plasma visfatin levels(r~2=0.101 P<0.01,r~2=0.247 P<0.01,r~2=0.318 P<0.05).Conclusion 1 The fasting plasma visfatin levels of patients with type 2 diabetes are elevated,and with the extension of the course,aggravation of pathological changes of complication,the fasting plasma visfatin levels are gradually increased,and the mean amplitude of glycemic excursions are significantly correlated with the fasting plasma visfatin levels.2 Visfatin is an adipocytokines,which is relative to lipid metabolism,and there is an important relationship between abdominal obesity and plasma visfatin,its levels may be affected by the amount of body fat,but it has a more close relations to fat distribution.
