Efficacy of IA Management on Patients with High Panel Reactive Antibody(PRA) before Kidney Transplantation.Objective : To probe the effects of immunoadsorption(IA) treatments on the patients with high panel reactive antibody(PRA) before kidney transplantation.Methods: 45 cases of Uremia patients with PRA >65% were divided into two groups randomly namely IA treatments group (23 cases) and Plasma exchang ( PE )group (22 cases). Blood pressure, Blood routine, serum immune globulin( Ig), complement (C3 ), PRA, thrombinum function and 5 hepatitis-related serum indexs were measured pre and post hemofiltration treatments respectively. The rejection and the functions of transplanted kidneys during 3 months post-transplantation were observed and other clinical indexs mentioned above were analysesed.Results: The Ig and C3 were decreased significantly (p<0.05) and PRA approached below 10% after PE or IA treatments. Compared with the PE group, the PRA level of the patients in IA treatment group was lower (P<0.05) and the treatment times were less than PE group, furthermore, the rejection rate in IA group was also lower than PE group.Conclusion: IA treatment on patients with high PRA before renal transplantation achieves better outcomes of kidey transplantation with fewer times of blood purification pre-operation and lower acute rejection rate after transplantation compared with PE management.
