Stenotrophomnas maltophilia is a nonfennentative,aerobic gram-negative bacillus that is widely distributed in natural and human-made environments.It has poor virulence,but it is recognised as a nosocomial opportunistic pathogen,especially in intensive care unit.The isolating rate is secondly to Pseudomonas aeruginosa and Acinetobacter baumanii.Clinical skin presentations caused by stenotrophomonas maltophilia include primary cellulitis,metastatic nodular skin lesions or cellulitis, gangrenous cellulitis,soft tissue necrosis,ecthyma gangrenosum and infected mucocutaneous ulcers.The infections pccurr in hematologic malignancies,chemotherapy and neutropenia,also in exposure to broad-spectrum antibiotics and presence of central venous catheter.Now we report 1 case of a farmer with skin ulcer caused by Stenotrophomnas maltophilia.The patient was without any risk factors except glucocorticosteroid for oral.The case is seldom,so we should pay attention to it.Wei Ji-bin,who is a 43 years old farmer of Linquan,Anhui province. The skin of interdigital space of his both feet had repeatedly appeared erythema,erosion,ulcer and effusion both with itch for 4 months,and this condition had aggravated for more than 20 days before his visit.We cultivated purulent secretion of both feet,and the stain was identified according to the colony and ATB bacteria identified meter.The result showed that the pathogenic bacterium was stenotrophomonas maltophilia.Patient's condition greatly improved after treated with 2 tablets SMZ-TMP bid for 3 weeks.The skin destruction was healed in the following 4 weeks but the interdigital space of feet was accretio and web-like for patient's misconduct.
