Objective:Neurogliocytoma which comes from neurepith- elium tissue is the most common malignant tumor in central nervous system. Now we take the complex therapy for the gliocytoma in clinic which include operation, radiotherapy and chemotherapy. But for the characteristic of invasion and recidivation of gliocytoma, the prognosis of the patient often imperfect, and the mortality is high. The grounding research of gliocytoma has because one of focal point and hot spot in neurosurgery domain in the past few years. During the invasive and metastatic process of gliocytoma, the loss of adhesiveness between tumor cells, the adhesiveness between tumor cells and cell matrix and the degradation of extracellular matrix(ECM) play the important roles. Matrix metalloproteinases(MMPs) is the emphasis in all the investigative protease. MMP-3 is the significant member in matrix metalloproteinases family. Adhesion molecule is one kind of transmembrane glycoprotein on cellular membrane. It plays an important role in the adhesion between cells and between cell and cell matrix. E-cadherin, a kind of adhesion molecule, distributes in adult epithelial tissue mostly. It is also the hot spot in the research of invasion and diversion of tumor recently. My purpose is to explore the effects of them in the process of invasion and diversion of gliocytoma through studying the expression and correlation of MMP-3 and E-cadherin protein in gliocytoma. Furtherly, we can explain the molecular mechanism of invasion in gliocytoma and provide certain theory for clinical therapia.Methods: (1) The case group involves fifty gliocytoma specimens from patients who were operated in neurosurgery in the second hospital of Hebei Medical University (thirty-one persons are male, nineteen femal, ages are between 18 and 76, average age is 45.32±16.51) and ten normal brain specimens from patients who are craniocerebral injury. According to the WHO(2000) separate criteria, two cases are grade I, twenty-th- ree cases are grade II, fifteen cases are grade III, and ten cases are grade IV. Grade I~II are seted in low-grade groupe, Grade III~IV are seted in high-grade groupe. We take immunohistoc- hemistry to detect the expression of MMP-3 and E-cadherin in normal brain tissue and gliocytoma, determin the percentage of positive cell and the magnitude of staining. (2) According to the percentage of positive cell and the magnitude of staining to count scores. Two persons count 1000 cells single-blind under the high power lens. According to the percentage of positive cell, we divide to: 0 score, <10%; 1 score, 10%~25%; 2 scores, 25%~50%; 3 scores, >50%. According to the magnitude of staining cell, we divide to: 1 score, poor; 2 scores, middling; 3 scores, strong. Plusing the positive percentage and magnitude scores is the specimen's IHC positive score: negative, 0~1 score, sign (-), weakly positive,2~4 scores, sign (+), strong positive,≥5 scores, sign (++). (3) We choose SPSS11.0 statistics software to process data, and use theχ2 text and Spearman rank correlation analysis to study the data. When P<0.05, the data has statistics significance.Results: (1) MMP-3 is negative or weakly positive in normal brain tissue. In gliocytoma, the brownish staining of endochylema and(or) cell membrane of tumor cells, blood vessel basilemma and endochylema of endotheliocyte cells is regarded as MMP-3 positive. With the aggravation of tumor, the percentage of positive cell increase. The positive staining rate of MMP-3 in gliocytoma is 78.00%(39/50).There is significant difference of MMP-3 expression between gliocytoma and normal brain tissue 10.00%(1/10), (χ2 =14.23, P<0.01). The positive staining rate of MMP-3 in low-grade gliocytoma (I~II grade) is 64.00%(16/25).There is significant difference of MMP-3 expression between low-grade gliocytoma and hihe-grade gliocytoma (III~IV grade) 92.00%(23/25), (χ2 =5.71, P<0.05). (2) E-cadherin expresses brownish staining on cell membrane and(or) endochylema near cell membrane in normal brain tissue strongly and steadily. In gliocytoma, the magnitude of staining in positive cell is weak and asymmetry, the positive rate become to weaken with the aggravation of gliocytoma. The positive staining rate of E-cadherin in normal brain tissue is 100.00%(10/10).There is significant difference of E-cadherin expression between normal brain tissue 100.00%(10/10) and gliocytoma 44%(32/50), (χ2 =8.37, P<0.01).The positive staining rate of E-cadherin in low-grade gliocytoma (I~II grade) is 60.00%(15/25), there is significant difference of E-cadherin expression between the low-grade gliocytoma 60.00 % (15/25) and the hihe-grade gliocytoma (III~IV grade) 28.00%(7/25), (χ2 =5.19, P<0.05). (3) The expression of MMP-3 and E-cadherin protein submit negative correlation in gliocytoma calculated by Spearman rank correlation analysis (rs=0.637, P<0.01). The result has statistics significance.Conclusion: MMP-3 is negative or weakly positive in normal brain tissue. The percentage of positive cell enhances with the aggravation of gliocytoma. It has significant correlati- on with pathological grade. E-cadherin is strong positive in normal brain. The percentage of positive cell decrease with the aggravation of gliocytoma.It also has significant correlation with pathological grade. The expression of MMP-3 and E-cadherin protein submit negative correlation in gliocytoma. They may take the synergistical effect during the invasive procedure of gliocytoma. The symphysial detection of MMP-3 and E-cadherin has a significant sense for judging the malignant grade and prognosis of gliocytoma.
