An Analysis Of Association Between Hyperuricemia And The Risk Factors Of Vascular Complications In Patients With Type 2 Diabetes Mellitus

Objective To investigate the relationship between hyperuricemia and incidence of vascular complications of type2 diabetes mellitus and analyze the possible mechanism.Methods Serum uric acid and other clinical data of 200 type 2-diabetes were collected. According to serum uric acid the patients were divided into high uric acid group(HUA) and normal uric acid group(NUA),the clinical indicators and incidence of vascular complications were compared between high uric acid group and normal uric acid group.Results(1) The prevalence of hyperuricemia was 16%in type 2 diabetes.(2) Hyperuricemia group showed higher Body Mass Index(BMI),systolic pressure(SBP),triglycerides(TG),lipoprotein a(Lpa),blood urea nitrogen(BUN),serum creatinine(SCr),fasting C peptide,fasting insulin(FINS),Homeostasis model assessment of insulin resistance(HOMA-IR),microalbuminuria(UALB),uric microalbumin excretion rate(UAER) and lower serum high-density lipoproteins cholesterol(HDL-C),apolipoprotein A(ApoA)than the group with normal uric acid(all p＜0.05),but there was no statistical significance in age,TC,LDL-C,FBG and HbA₁c(p＞0.05)(3) Serum uric acid was positively associated with BMI,SBP,TG,Lpa,BUN,SCr,Fasting C-Peptide,FINS,HOMA-IR,UALB and negatively associated with HDL-C,ApoA level.(4) more patients with coronary artery disease(CHD),cerebral infarction(CI),Diabetic Nephropathy(DN),Diabetic Retinopathy(DR) in high uric acid group than in normal uric acid group(p＜0.05).Conclusion(1)The level of blood uric acid in type 2 diabetes was closely correlated to obesity, insulin resistance,dyslipidemia,hypertension and decrease in renal function.(2) type 2 diabetes with Hyperuricemia showed higher incidence of vascular complications than those with normal uric acid.
