The Study Of The Clinical Experiences Of Li Saimei In Diagnosing And Treating Type 2 Diabetes Mellitus Compl Icated With Hyperur Icemia

ObjectiveHyperuricemia is a metabolic disorder of purine and plays an important role in the occurrence and development of diabetes mellitus and other metabolic diseases.Type 2 diabetes mellitus and hyperuricemia as two vital components of metabolic disorders are closely related to each other,they promote,influence each other by participating in the development of some chronic compl ications including diabetic nephropathy,coronary heart disease and peripheral neuropathy.Therefore,if early intervention can he implemented,which reflecting the idea of treating t he disease before the occurrence" in Chinese med icine,these chronic complicat ions can be reduced or avoided.Professor Li Saimei has been engaged in clinic al research of endocrinology and treating diabetes for a long time and hence she has accumulated a lot of eliniical experience.Based on the generation of attribuion partial order structure diagram and knowledge discovery method,this study aims to explore the cl inical experience,prescript ions rules and academic thoughts of Professor Li Saimei in the treatment of type 2 diabetes mellitus complicat ed wi th hyperuricemia.MethodsThis study collected and sorted out patients with type 2 diabetes mellitus complicated with hyperuricemia from March 2018 to December 2018 in the clinic of Professor Li Saimei,at the South Building of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine.After screening the medical records that meet the inclusion criteria and exclusion criteria,the data is standardized and the Txt format database is established for each patient’ s case information,syndromes,symptoms,tongue,pulse,main prescription,and herbs.After that,the information for each database is analyzed by the non-statistical partial order structure pattern of formal concept analysis.In this study,each case was taken as the object,while patient’ s gender,syndrome type,symptom,tongue,pulse,main prescription and herbs composition as attributes,Through the establishment and optimization of formal background,the generation of attribution partial order structure diagram and knowledge discovery method,each data has been analyzed with multi-level and multi-angle,and finally the clinical experiences of Professor Li Saimei in the treatment of type 2 diabetes mellitus complicated with hyperuricemia have been summed up.ResultsThis study included 190 cases of definite diagnosis of type 2 diabetes mellitus with hyperuricemia,including 115 male patients and 75 female patients.Among 190 cases collected in this subject,a total of 33 types of syndromes are involved.From the perspective of six meridians syndrome differentiation,all involve Taiyang syndrome,Shaoyang syndrome,Yangming syndrome,Taiyin syndrome,Jueyin syndrome,Shaoyin syndrome,especially Yangming syndrome and Shaoyin syndrome are more common.From the perspective of viscera syndrome differentiation,the spleen and stomach,liver and kidney are the main ones.From the point of view of disease,damp-heat syndrome is the most common,and also included qi-deficiency syndrome,yang deficiency syndrome,qi stagnation syndrome,blood stasis-heat syndrome,and phlegm-heat syndrome.Combined those symptoms and signs,tongue and pulse of the patient,the symptoms of Yangming and dampness-heat are more prominent.The common symptoms of the patient are:thirsty,insomnia,sweating,yellow greasy tongue coating,taut and slippery pulse.Sijunzi Decoction and Gegen Huangqin Huanglian Decoction are two main prescriptions used by Professor Li Saimei to treat type 2 diabetes mellitus complicated with hyperuricemia.Sijunzi Decoction mostly combined with those prescriptions which have actions of heat-clearing,warming yang,kidney tonifying,clearing the heat and warming the cold,soothing the liver and promoting blood circulation.Among them,it is most common combined with prescriptions that act to clear heat or clear the damp heat;Gegen Huangqin Huanglian Decoction often combined with prescriptions which have actions of strengthening the spleen,soothing the liver,heat clearing and cold warming,promoting blood circulation,replenishing liver and kidney.Among them,it is most commonly combined with prescriptions that act to strengthen the spleen.The most commonly used group of herbs are:Zhigancao,Yinyanghuo,Chishao,Yumixu,Huanglianpian,Huangqinpian,Gegen,Ganjiang,Fuling,Shudangshen,Shengdihuang,Tianyeju,Tianhuafen,Cangzhu(the corresponding prescription included Gegen Huangqin Huanglian Decoction,Sijunzi Decoction,Lizhong Decoction).ConclusionProfessor Li Saimei’ s clinical experiences in the treatment of type 2 diabetes mellitus complicated with hyperuricemia are as follows:①syndrome differentiation is mainly based on six meridians syndrome differentiation,combined wi th syndrome different iation of viscera and syndrome differentiation of qi and blood;②grasping the main symptom,the effect has the right to speak;③ the combination usage of ancient prescriptions with modern prescriptions;④treatment with pure Chinese medicine as soldiers,Western medical examination indicators as guidance;⑤pay attention to the Speech therapy,pay attention to the psychological const ruction of patients.Based on the knowledge discovery method of non-statistical partial order structure model,this topic comprehensively discusses thinking and academic thoughts of Professor Li Saimei s on the t reatment of type 2 diabetes wi t h hyperuricemia.It is believed that i t can provide some reference for t hose Chinese physicians i n clinical treatment.
