Objective:(1)By detecting the expression of telomerase reverse transcriptase mRNA in oral lichen planus and oral squamous cell carcinoma, reveal the relevance of TERT mRNA expression in oral lichen planus and oral squamous cell carcinoma.(2) To investigate the expression of telomerase reverse ranscriptase mRNA in different organizations which that normal oral mucosa,oral lichen planus and oral squamous cell carcinoma ,and to evaluate the role in OLP carcinogenesis and the incidence of OSCC,in order to monitor the high-risk groups of cancerous OLP.Methods: TERT mRNA expression was analysed in 20 cases of OSCC, 20 cases of erosive OLP, 20 cases of non-erosive OLP and 12 normal oral mucosa using an RNA in situ hybridisation method. The study analysised the correlation between the expression of TERT mRNA and OLP, as well as the role in the process of cancer in OLP and OSCC.Results: (1)Expression of TERT mRNA progressively increased from normal oral mucosa to non-ersive oral lichen planus, erosive oral lichen planus, and OSCC. The positive rates of TERT mRNA in normal oral mucosa and non-ersive oral lichen planus were 8.33%(1/12) and 15%(3/20) respectively,and There was no significant difference between them(P>0.05).The positive expressing rates of TERT mRNA in erosive oral lichen planus tissue was 45%(9/20). Furthermore,there was a significant difference in the expressing level of TERT mRNA among the cases with erosive oral lichen planus and those with OSCC,non-ersive oral lichen planus and normal oral mucosa (P<0.05). Expression of TERT mRNA in OSCC was 80%(16/20),it was significantly greater than erosive oral lichen planus,non-ersive oral lichen planus and normal oral mucosa (P<0.05). (2)Staining intensity of TERT mRNA was much lower in the normal oral mucosa and non-ersive OLP than erosive OLP and OSCC(P<0.05). (3)There was not correlation between the positive rates of TERT mRNA in OLP with the age of patients with OLP.(4) there was not significant difference of the positive rates of TERT mRNA in OSCC with different histological classification.Conclusion: (1) there was weak of TERT mRNA in non-erosive OLP and normal oral mucosa. It Show that the epithelium of non-erosive OLP and normal oral mucosa had a lower proliferative capacity.(2) Expression of TERT mRNA in erosive OLP was significantly greater than non-ersive oral lichen planus and normal oral mucosa. Erosive OLP are in a higher state of proliferation than non-ersive OLP and have higher Potential of malignant transformation. (3) there was not correlation between the positive rates of TERT mRNA in OLP with the age of patients with OLP.(4) TERT may play a crucial role in the development of OSCC.
