Objective:To investigate the role of CXCR4 and P-Akt in the genesis of oral squamous cell carcinoma by means of evaluation on their expression and distribution in oral lichen planus,leukoplakia and squamous cell carcinoma.Materials and Methods:The expression and distribution of CXCR4 and P-Akt were studied in 30 cases of oral lichen planus(OLP), 30 cases of oral leukoplakia(OLK),30 cases of oral squamous cell carcinoma(OSCC)and 10 cases of normal oral mucosa(NOM)by SP immunohistochemistry with the CXCR4 rabbit anti-hunam polyclonal antibobody and Phospho-Akt mouse monoclonal antibody(Ser473).The rate of positive reaction was observed by image analysis system,and the average staining intensity(gray-level)was analyzed using Image-Pro Plus software.Results:1)Significant CXCR4 and P-Akt immumoreactivity were seen in OSCC,moderate expression in OLP and OLK,and negative expression in NOM.The diference of average staining intensity was statistically significant.No significant diference in expressions of CXCR4 and P-Akt was found between OLP and OLK(P>0.05).2)The positive rate of CXCR4 and P-Akt in OSCC were 63.3%and 70.0%,and expressions of both were higher than NOM,OLP and OLK (P<0.05).The positive rate of CXCR4 and P-Akt in OLP were 36.7%and 43.3%,and that in OLK were 33.3%and 36.7%.There was no significant diference between the positive rate of CXCR4 and P-Akt in OLP and OLK(P>0.05),which were higher than NOM(P<0.05).3)The expressions of VEGF and CXCR4 in OSCC were correlated significantly with lymph node matastasis and the degree of tumor differentiation(P<0.05),and their expressions were not associated with patient's age and sex(P>0.05).4)The expression of CXCR4 was significantly related with the expression of P-Akt(r=0.574,P<0.01)in OLP,OLK,OSCC and NOM.Conclusion:1)The abnormal expressions of CXCR4 and P-Akt are correlated with the canceration of OLP and OLK.2)OLP and OLK that highly expression CXCR4 and P-Akt have a tendency to deteriorate,which should be closely follow up.3)The expressions of CXCR4 and P-Akt are closely related to lymph node metastasis and the degree of tumor differentiation in OSCC, which may be used to evaluate prognosis.4)SDF-1/CXCR4-PI3K/Akt pathway may act as an important role in the oncogensis of OSCC.
