ObjectsIn order to identify the microdissection of the pineal region, we investigated dead body treated by the perfusion of vascular . In particular, associated with vein and arachnoid membrane. Simultaneously, we made a surgery experience summary on occipital transtentorial approach by resecting the pineal region tumor, to enhance the surgery therapeutic efficacy of the pineal region tumor.MethodsThis study was divided into two parts. The first part for microdissection of the pineal region. The pineal region related vein, venous clearance, the enveloping relationship with the veins and arachnoid membrane were observed under in the X10-surgery microscope. The mainly related vein diameter and length of the pineal region were measured. The second part was to review the occipital transtentorial approach surgical resection of 21 cases of pineal region tumor patients clinical data, intraoperative video and early postoperative follow-up results , and made a summary and analysis. These cases were collected from 9, 2006 to 9,2008 in my hospital.Results1.The Galen's venous system thickening of the arachnoid membrane surface is the double arachnoid membrane enveloping the Galen's venous system trunk and its branches. Galen's venous, in the last paragraph of the basal veins of bilateral, bilateral the internal cerebral veins like a hood at the last paragraph of the pineal region of the open top of the " five calw" wrist vein into the brain are tentorial dome cacumen and brain sickle confluence.2. 21 cases of surgery in patients with pathologically confirmed 17 cases of germ cell tumors, 3 cases of glioma , 1 cases of pineal cyst. Because of intracranial hypertension due to hydrocephalus, there were two cases of ventricular drainage and two cases of ventriculoperitoneal shunt. 5d postoperative MRI within 19 cases reviewed, CT reviewed two cases, showing all tumors resection totally. All the patients discharged from hospital after a good recovery, the disappearance of intracranial hypertension symptoms, hearing and masonic exercise improved, but there are still 7 cases of vertical eye movement disorder, fuzzy vision example 2.ConclusionsThe experiment confirm that the Galen's venous system thickening of the arachnoid membrane surface is the double arachnoid membrane enveloping the Galen's venous system trunk and its branches. Occipital transtentorial approach by surgical resection of pineal region tumor horizons open, exposed to a wide range of direct vision at protecting deep vein, timely treatment of the tumor feeding arteries, to the removal of any suitable size and the growth direction of pineal tumors, particularly suitable for large tumors and pediatric patients.
